I have the following comments. 1. The existing 4-inch sewer force main stub is privately owned and therefore a connection to it cannot be made. An alternative sewer connection must be designed including the connection to the town's public main line sewer. If the applicant secures the rights to use the 4"pipe,a manhole should be installed where the connection is made with the ability to launch a sewer pig for maintenance. 2. The sewer connection fee for a residential unit in this area is$1,557. The connection fee for a commercial unit is$3,114. 3. The water main in Sharpners Pond Road is not shown correctly. A 12"-inch main exists in Sharpners Pond Road that the applicant may connect to. Another option may be to connect to the 12-inch water main in Turnpike Street. 4. The connection fee for a 4-inch domestic water lines is$10,000. Very truly yours, Timothy J. Willett Operations Manager CC: Bruce Thibodeau, Director i 0' � I i 1 � I Noah Andover Health Department Community and Economic Development Division i To: Jean Enright,Interim Town Planner From: Susan Sawyer,Health Director Date: December 10,2015 Re: 2302 Turnpike Street This memo is in response to the application before the planning board regarding a proposed commercial operation at 2302 Turnpike Street.The Health Department has the following comments. 1) The proposal shall comply with the North Andover Board of Health regulation regarding the dumpsters proposed on the site.At minimum dumpsters must be placed over 20 feet from any structure;placed on a pad and enclosed by a fence. 2) The proposal shall not create a new nuisance noise or nuisance odors that could affect the enjoyment of neighborhood properties.Addressing the potential issue before it occurs is critical to the health and welfare of the public.Note that nuisance is a difficult issue where residential property is close to commercial properties. 3) The property is currently serviced by an onsite subsurface disposal system.The applicant proposes that the new building will be connected to municipal sewer.The onsite septic system must be abandoned properly in accordance with MA DEP regulation,310 CMR 15.354. 4) The property is currently serviced by a private well.Abandonment of this well shall meet the requirements of section 6 of the North Andover Board of Health regulations regarding wells. The Division has determined that the proposed project is located within the mapped Priority and Estimated Habitat of Blandilg's Turtle (E)ttydoidea blandingii), a species state-listed as Threatened. The purpose of the Divisions review of the proposed project under the WPA regulations is to determine whether the project will have any adverse effects on the Resource Areas Habitats of state-listed species. The purpose of the Division's review under the MESA regulations is to determine whether a Take of state-listed species will result from the proposed project. I offer the following comments: 1) Tim Willett from Water and Sewer Dept. has rejected both the water and sewer services as shown. The applicant needs to redesign and submit to him for approval. 2) The Southern driveway entrance is approx' a y 70' from the intersection with Turnpike St. It should be moved north to approximately t e 48' ontour(flat area)to maximize distance from intersection. We will require a Fire Hydrant to be within 500 feet from the proposed development. We will require the mulch storage be permitted through the fire department yearly and all mulch storage will be in accordance with NFPA-1. We will also require permits for any and all flammable and combustible fluids that are stored on the site to be in accordance with the Mass general law permit schedule. We will want 360 degree access around the building for all fire apparatus to include the ladder truck.
