HomeMy WebLinkAbout20160105 As-Built Plan - As-Built Plan / Installation Certificate Form - 254 GREAT POND ROAD 5/31/2018 Sullivan Engineering Group, LLC Civil Engineers& Land Development Consultants May 31,2018 Town of Not Andover Planning Board Re: 254 Great Pond Road, North Andover Compliance with Watershed Special Permit Modification To Whom It May Concern: In accordance with the Watershed Special Permit Decision on 1/5/2016 and the Watershed Special Permit Modification on 4/19/2016 (recorded in Book: 14660 Page 8) this letter is prepared to state that the site has been constructed in substantial compliance with the approved plan, decision, and modification. The 1/5/2016 decision permitted a 22' x 23' single story addition with associated drainage mitigation and site grading. The 4/19/2016 modification allowed for a 309 s.f. garage expansion, new deck, drainage mitigation, site grading, and driveway resurfacing. All the work onsite is completed and the site is fully stabilized by landscaped means. The driveway has been resurfaced and the drainage flow is noted on the as-built plan to shed mostly onsite(with only limited driveway surface draining to Great Pond Road) An As-built plan has been prepared by this office dated 5/31/2018, scale 1"=20' that reflects the as-built condition of the additions, driveway, drainage infrastructure, and site grading.
