The following items have been found to be in non-compliance with Title 5—the State Environmental
Code.
The onsite wastewater system serving this propoerty was found to have be failing pursuant to 310
CMR 15.303 (1) by a System Inspector in a report dated July 28, 2007. An upgrade to address this
problem is required to have been completed pursuant to 310 CMR 15.305(1) and the timeframe for
completing such an upgrade has passed with no evidence available to indicated this has been
completed
I hereby certify that the following action(s) be taken within 90 days to complete system
upgrade
And that you inform this office when those actions have been completed.
Please be advised that failure to perform the specified actions may result in further enforcement
action
