We have revised the plan set per your comments which are reproduced below in italics with our response below in bold. Sheet 1 of2.- 1. On PLAN, indicate the legal boundaries (property bearings) of the facility to be served(310 CMR 15:220(4)(a)). Response:The legal boundaries depicted on the plan were derived from the latest deed for the property recorded in the N.E.R.D. in Book 8230,Page 290 and the referenced plan, dated March 1970 and recorded at the N.E.R.D., on April 17, 1970 as plan No.6179. A copy of each is attached for your reference. 2. On PLAN, indicate distances from septic tank to nearest property line and wetland flag 4B (NA 3.2j,1). Response: The distance from the septic tank to the property line of 27 feet and the distance from the septic tank to wetland flag 4B of 140f feet have been added to the plan. Note that the distance from wetland flag 20A to the septic tank is approximately 120f feet. 3. On PLAN, indicate location of the foundation drain for the new building(310 CMR 15.220(4)(m)) and(NA 3.2k&1.). Response: A proposed foundation drain,with an outlet invert of 121.0, has been added to the plan and legend. The drain is shown as a dashed line around the structure(except at garage entrance) and labeled with "FD" in the back yard extension. 4. On PLAN, indicate location of the existing onsite wastewater system to be abandoned (NA 3.2q.). Response: The location of the system is unknown and no record as-built was found on file at the Health Department. In Note 5, after the words: IN ACCORDANCE WITH TITLE 5, add the citation: 310 CMR 15.354 (3)(a-c), and change the last word in the paragraph: REGULLTIONS to read: REGULATIONS. Response: The spelling error has been corrected. 6. On PLAN detail, at the east end of the PVC membrane, confirm the top elevation of the membrane is not exposed above finish grade where the 127 contour is proposed. Response: The top of the membrane is below grade by approximately 4 inches at this location 7. On DEEP OBSERVATION HOLE LOG, T-5, in DEPTH(In.) column, change the numbers: 34-96"to read: 80-99". Response: This test pit log for pit 5 was corrected, as noted. 8. On PLAN detail, in lieu of the words: REMOVE ALL FILL & TOPSOIL WITHIN 5'(7_YP.) OF THE ABS TRENCHES, clarify the soil to be removed to include the FILL, A and B layers of existing soil underneath the soil absorption system. Response: The note was amended to include topsoil, subsoil and soils underneath and five feet around. Sheet 2 of 2: 1. On septic system components, indicate they shall be marked with magnetic tape or a comparable means in order to locate them once buried(310 CMR 15.221(12)). Response: Note 1 has been added to this sheet requiring the magnetic tape. 2. On PROFILE detail, after the notes: 10'4"DIA. SOLID SCH. 40 PVC, S = 0.020 and 18'4"DL4. SOLID SCH. 40 PVC, S= 0.0183, add the words: WITH WATERTIGHT JOINTS(310 CMR 15.222 (3) & (4). Response: These water-tight joint notations were added to the profile on the revised plan. 3. On PROFILE detail, after the notes: 10'-4"DIA. SOLID SCH. 40 PVC, S = 0.020, add the words: WITH WATERTIGHT JOINTS-INSTALL ON COMPACT, FIRM BASE AND ON A CONTINUOUS GRADE INA STRAIGHT LINE(310 CMR 15.222 (5) &(7)). Response: Note 3 has been added to this sheet requiring the notation above. 4. On 1500 GALLON CONCRETE SEPTIC TANK(MONOLITHIC POUR)-SECTION detail, add a note or other description: INLET AND OUTLET TEES SHALL EXTEND A MINIMUM OF 6 INCHES ABOVE THE FLOW LINE OF THE SEPTIC TANK(310 CMR 15.227(1)). Response: A note requiring the tees to extend a minimum of six inches above the flow line was added to the detail. The flow line was also labeled on the revised detail. 5. On 1500 GALLON CONCRETE SEPTIC TANK(MONOLITHIC POUR)-SECTION detail, clarify which product is to be used. a gas baffle or an approved tee filter. If an effluent tee filter is to be provided,provide current MA DEP issued Approval Letter for selected product, provide a note to install it per manufacturer's specifications, indicate filter type/name to be noted on manhole cover,provide riser with manhole cover at grade placed over filter and specify annual filter maintenance (310 CMR 15.227(7)) and(NA3.3). 2 of 4 P:\18\18-60\docs\leners\2019-05-13 BOH response letter.docx andover consultants inc. Response: The revised detail depicts a gas baffle; the notation of approved filter has been deleted. 6. On 1500 GALLON CONCRETE SEPTIC TANK(MONOLITHIC POUR) -PLAN detail, after the words: IRON FRAME AND COVER TO GRADE, add the words: SECURED TO PREVENT UNA UTHORIZED ACCESS(TYP.) (310 CMR 15.228(2)). Response: The frame and cover note was revised per the notation above. 7. On 1500 GALLON CONCRETE SEPTIC TANK(MONOLITHIC POUR)-SECTION detail, after the words: DESIGN FOR H2O LOADING, add the words:ALL OPENINGS AROUND TANK PERIMETER AND AT THE TOP OF TANK, EXCEPT COVERS, SHALL BE WATERTIGHT(CMR 15.221 (1)). Response: The design for H2O design note was amended to include the water-tight notation cited above. 8. On CONCRETE DISTRIBUTION BOX detail, add the note: INVERT ELEVATION OF ALL OUTLETS SHALL BE EQUAL TO EACH OTHER (CMR 15.232 (3)(b)). Response: The distribution box detail was revised by adding a notation that the outlet inverts shall be installed at the same elevation. 9. On CONCRETE DISTRIBUTION BOX detail, in lieu of the words: 6"GRAVEL,provide the words: CONSTRUCTED LEVEL ON A MECHANICALLY COMPACTED 6"AGGREGATE BASE(CMR 15.221 (2)). Response: The distribution box detail was revised by adding a notation cited above. 10. On PROFILE detail, in lieu of the words: REMOVE ALL FILL & TOPSOIL WITHIN 5'(TYP.) OF THE ABS TRENCHES, clarify the soil to be removed to include the FILL, A and B layers of existing soil underneath the soil absorption system. Response: The note was amended to include topsoil, subsoil and soils underneath and five feet around. 11. On ABSORPTION TRENCH SECTIONS detail, after the words: BACKFILL MATERIAL, add the words: ON TOP OF THE SOIL ABSORPTION SYSTEM SHALL CONFORM TO 310 CMR 15.240(9) and extend the arrow to a graphic location above the 2"washed crushed stone layer, and add a note: FILL MATERIAL MEETING SPECIFICATION OF 310 CMR 15.255(3)SHALL BE PLACED ALONG THE VERTICAL SIDES, BETWEENAND UNDERNEATH THE CRUSHED STONE TRENCHES. Response: The section note was revised and the arrow extended per the comment above. A note was added for systems,when in fill,to provide the soil specified per 310 CMR 15.255 between and underneath per the above. 12. On SECTION and LONGITUDINAL SECTION details, after the words: PROP. FINISHED GRADE and FINISH GRADE respectively, add the words: GRADE OVER LEACH FIELD MIN. SLOPE 0.02 FEET PER FOOT. Response: The 2% minimum grade notation was added per the above comment. 13. On LONGITUDINAL SECTION and PLAN OF ABSORPTION TRENCHES details, after the words: 4"DIA. PERFORATED SCH. 40 PVC PIPE, add the words: WITH ORIFICES 3 of 4 PAI 8\1 8-60\docs\letters\2019-05-13 13011 response letter.docx /J�,andover consultants inc. EVENLY SPACED ALONG 2 ROWS RUNNING THE LENGTH OF THE LINE, ON EACH SIDE, MIDWAY BETWEEN THE INVERT AND CENTER LINE WHICH SEPARATES THE UPPER AND LOWER HALVES OF THE PIPE. THE ORIFICES SHALL BE NO SMALLER THAN 318 IN. AND NO LARGER THAN 518IN. IN D)AMETER (310 CMR 15.251 (8)). Response: Note 2 has been added to this sheet to include the information noted above. 14. On ABSORPTION TRENCH SECTIONS details, delete the graphic indications and words related to: UNTREATED BUILDING PAPER and provide for cover over the trenches as authorized in Title 5. Response: The building paper has been removed, as directed. In addition: 1. On both sheets, delete references to ABS(acrylonitrile-butadiene-styrene) and indicate all piping to be minimum Schedule 40 PVC(NA 3.2d.). Response: The ABS on the plans is intended to be shortened "absorption", not ABS pipe. The plan was revised to change 'labs trench" to "trench". 2. Submit MA DEP soil evaluation forms 11 & 12 (310 CMR 15.018 (2)) and(NA 2.3). Response: The soils were tested on March 30,2006 by a soil evaluator with Merrimack Engineering Services and witnessed by Inspector A. McBrearty for a proposed system designed by Merrimack Engineering Services. Since the proposed system was not installed and the area of the system has not changed,I made a phone call to Director LaGrasse on November 5,2018,to determine if the 2006 data was ok to use for the design and told the test data was ok to use(as is typically allowed when the site conditions have not changed since the testing). The 2006 test results are reproduced on our plan in table format,sheet 1 of 1. Please find attached 3 copies of the revised plans, revision dated May 13, 2019 for your review. A few items of note: 1. There is a typo which we noticed on our second review but did not pick up on our first review. It is a small matter but should be corrected. In the pert test logs on Sheet 1 they indicate the testing was done in 2016, when it was actually done in 2006. Perhaps the engineer can come in an change the date on the plan and then initial the changes on the plans on file at the office? 51'a I Lo `ot// �� � �,�-���' 549 r �.� � 6r`.1— ot- 2. The second issue is that it seems the engineer has over designed this system -which he is 100% authorized to do and there is nothing wrong with that so it is not a reason for plan disapproval during our review. We wish to call this to your attention in case you wish to, or wish us to, communicate with the design engineer about this. The reason I say this is because the assessor's records indicate the current house contains 4 bedrooms. If that is the case, they are planning to replace a 4 bedroom house with a new 4 bedroom house which in the eyes of Title 5 is considered an Upgrade. Therefore, if the assessor's information is correct, since it is an Upgrade there is no need to design or build a reserve area. This could potentially save considerable expense during the construction process and might be something the engineer would want to research. Otherwise, everything is in compliance with regulatory standards. Let me know if you'd like us to outreach to the design engineer about these two items.
