The proposed work includes replacing the support footings under an existing 10'by 20'sunroorn with four FZ Post helical piers. The closest side of the sunroom is 55'from Bordering Vegetated Wetlands (BV v). j During the August 12,2020 public meeting, the North Andover Conservation Commission voted unanimously to approve this project. The project is approved only as it relates to the Conservation j Commission and any and all other approvals should be obtained by the applicant,/homeowner. Open slotted decks,without roofs, equal to or less than 300 square feet surface area, inclusive of stairs and ramps,which are to be attached to an existing home,where soil disruption during construction is limited to less than 50 square feet,and where no permanent soil alterations are planned under the footprint of the deck El B. The local Wetland Bylaw filing fee of$25 has been included with this application. i A written narrative description/scope of work of the proposed project and potential impacts to all wetland resource areas has been included. ❑ The applicant has identified and clearly marked all wetland resource area boundaries in the field. Two (2) copies (and an electronic copy when possible) of a 1" = 20' scale (or other as deemed appropriate by the Conservation Commission or agent thereof) drawing showing existing and proposed conditions and the delineated wetland resource areas (see plan requirements below for additional detail). Plan ReqWrements (Section 4.4.3 B): Certified and/or professionally prepared plans are not required. However, the North Andover Conservation Commission or its designated agent shall have the final authority to determine whether the submitted plans/drawings are adequate and include all pertinent information for review. At a minimum, all plans submitted shall clearly and accurately show the present conditions and proposed work and depict the location of all wetland resource areas within 100' of the proposed work. All plans must be signed and dated by the person who prepared them. The Commission may also require the following information on the proposed plans in addition to the aforementioned criteria where applicable: El Location of any subsurface sewage disposal systems. r] Locations of any drainage structures. El Locations of any easements,including but not limited to, drainage and utility easements. El Locations of any open space boundaries if(e.g. if the lot is affected by the 50' open space boundary as required in a Planned Residential Development(PRD)), El Location and nature of all proposed alterations to the Buffer Zone Resource Area. Concrete and Cement Products-If any concrete or cement is installed as part of this project,Contractor warrants only that the material is of the proper mIx,strength and placement for one(1)year.There Is no warranty of any kind against cracks or discoloration.All concrete will crack,and'the presence of cracks does not indicate a problem. Contractor cannot match new cement or concrete to,the color of existing concrete.Color variations wil I occur,and wil I change with time,and Contractor makes no guaranty as to color. Exclusions-THIS WARRANTY DOES NOT COVER,CONTRACTOR SPECIFICALLY DISCLAIMS LIABILITY FOR,AND CUSTO MER HOLDS CONTRACTOR HARMLESS FROM: 1)exterior waterproofing,unless provided by additional warranty,2)plumbing or electrical damage;3)Customer•caused damage;4)dust from installation:5)damage to real or personal property such as walls,countertop,or floor coverings,framing,sheetrock,exterior materials,cabinets,appliances„and so on,including any damage alleged to have been done by the Contractor's use of heavy equipment necessary to complete the job;6)any injury or damage caused by mold to property or person;7)failure or delay in,performance or damage caused by acts of God(flood,fire,storm,methane gas,etc.),acts of civil or military authority,or any cause outside Contractor's control;8)damage from a lifting operation;9)basement water seepage;10)damage from heave,lateral movements/forces of hillside creep,land sliding,or slumping of fill soils;and 11)settlement due to organics or other causes such as sink holes below the slab,While Contractor assumes responsibility for utility damage that occurs as a result of Contractor's installation„such protection is limited to relpaci ng/repair the area Contractor damaged,does not include damage to unmarked private lines,and does not include any upgrades to utilities for code compliance or other reasons. Service calls made that are found to be of no fault due to a defective system may be charged a service fee.Contractor's liability under this warranty shall be limited to the replacement of any defective work or material,and contractor shall be liable for no other damages or losses,direct or incidental. Items For Which Customer Is Responsible-Customer shall:1)make full payment to the crew leader upon completion of work:2)prepare the work area for installation;3)be responsible for any finish carpentry,painting,paneling,Iandscaping,etc-that may be necessary after Contractor's work is finished::4)mark private lines(sewer,satellite, propane,sprinkler,etc.)5)maintain positive drainage away from the repaired'wall(s):,6)keep gutters clean and in good working order;7)direct downspouts a sufficient distance away from the repaired wall(s):8)maintain proper expansion joints In concrete slabs that are adjacent to the repaired wal I(s);and 9)any items mentioned in this Contract under 'Customer Wilt'or"Additional Notes.' EZ Post EZ Post Helical Piers-Contractor warrants the helical deck piers to be stable against settlement for 10 years from the date of installation,or else Contractor WH provide the labor and materials,at no cost to Customer,to correct the problem with the foundation piers.Contractor does not warrant to lift,to close cracks,to renderdoors and windows operational,or to move walls back to their original position,but will do its best to,achieve positive results In this regard.Customer should be aware that damage can occur to the structure during a I ift operation and that Contractor is not responsible for such damages.Foundation piers provide vertical support only and cannot be expected to provide lateral support.If lateral movement occurs,additional work may be needed at an additional cost.Height adjustments made after Installation which are not the result of pler settlement can be made,but will be subject to a service charge.
