Pre-Inspection phone Interview completed on 10/28/20 with Cindy-
Green Apple is open for outdoor dining, indoor dining and take out. Two tables on porch and some
in driveway. Using barriers. Menu has been reduced. Signage and face coverings used. They have
tables six feet apart and a few dividers. Using Lysol and disinfectant spray. They provide sanitizer.
Silverware is wrapped, not pre set.
On site inspection 10.30 at 8:00 AM revealed -
Establishment has covid policies in place. Staff wearing mask. Sanitizer disinfectant and test strips
available. New stove appears commercial. Tables are 6 ft apart. No evidence of pests.
