Dear Owner;
The North Andover Health Department personnel conducted an authorized inspection
of your property at the above referenced address on 34 Main Street, Apartment 4, North
Andover MA, 01845, in response to a complaint filed with this Department. The inspection
revealed violations of the State Sanitary Code, Chapter II as listed on the attached Violation
Form.
You are hereby ORDERED to correct the violations within the time allotted on the
enclosed form. Failure to comply within the specified time period will result in a fine of up to
$500 per day' in accordance with 105 CMR 410.910 of the State Sanitary Code. The fine will
continue to accrue until the subject property is brought into compliance with this Order to
Correct. Each day or portion thereof during which the violations continue shall constitute a
separate offense.
You have the right to request a hearing before the Board of Health if you feel this Order
to Correct should be modified or withdrawn. A request for said hearing must be made in
writing and received by the Health Department within seven(7) days from receipt of this
Order. At said hearing you will be given an opportunity to be heard and to present witnesses
and documentary evidence as to why this Order should be modified or withdrawn. All affected

attorney. You have a right to inspect and obtain copies of all relevant records concerning the
matter to be heard.
If the dwelling unit is vacant or becomes vacant before the violations are corrected,the
dwelling unit cannot be re-rented or re-occupied prior to compliance and prior to an inspection
by the North Andover Health Department in accordance with 105 CMR 410.010(A). Please call
the North Andover Health Department at(978) 688-9540 for an inspection. An inspection
performed by the North Andover Health Department is required.
If you have any questions,comments or concerns,please feel free to call me between the
hours of 8:00-4:30 on Monday,Wednesday and Thursday,8:00-6:00 on Tuesday and 8:00-12:00

Lead paint determinator must be brought in and, if noted, all
lead aint must be properly remediated.
410.500 Closet inside the living room area has damage to the ceiling 5 Days N
with insulation leaking out of it and into entire closet. Repair
this area and clean so it is free of loose insulation.
410.500 Bedroom ceiling has a leak and caused portion of ceiling to 5 Days N
collapse. Investigate and repair leak, and repair all water
damage caused by leak to ceiling and surrounding area.
410.351(A) Front porch is missing light bulb. Replace light bulb. 15 Days N
410.480(C) Dwelling unit has over 3 units inside building. Front main 15 Days N
entrance must self-close and lock automatically. Repair or
replace door so it is self-closing and self-locking. Provide all
residents with keys to this door as well.
410.481 No posting of owner's contact information is apparent in this 30 Days N
unit. Post owner or managing partners name, address and
telephone number in compliance with 410.481
410.351(A) Bedroom has outlets that are not working or not working 10 Days N
properly.Repair all outlets to function properly.
410.500 Bedroom wall shared with living room has water damage 30 Days N
noted and is misshaped. Repair all water damage and
bedroom wall so it is smooth and cleanable.

410.501(B) Door to bedroom from outside roof deck is not weather tight. 10 Days N
Water leaks in when it rains and gap is present. Repair door
so it is weather tight.
410.602(D) Bags of trash and other bulk debris present on roof outside of 10 Days N
bedroom. Clean and dispose of all trash or debris properly.
410.500 Sink has a massive hole in the wall underneath the counter. 15 Days N
Repair wall so it is free of hole.
410.500 Closet wall in bedroom area is cracked and damaged. There 30 Days N
is a hole in the ceiling tile above the close as well. Repair
ceiling the and wall.
410.351(A) Thermostat in the unit is not working.No method of heat 10 Days N
control present in this unit. Occupants stated heat is
controlled by downstairs unit. Repair heat so it can be
controlled by this unit.
410.551 Living room has many windows without screens. Provide 30 Days N
screens free of tears/defects in all windows.
410.351(A) Living room has multiple outlets that are not functioning 10 Days N
correctly. Repair all outlets to function properly.
410.280(B) Bathroom fan is not working properly. Fan is missing light 10 Days N
cover and has dust/dirt build up. Repair and clean fan.
410.351(A) Oven is not working properly. Repair oven to function 15 Days N
properly.
410.501(B) Front door has a gap under the door and is not weathertight. 15 Days N
Repair door to be weather tight.
410.351(A) Multiple light fixtures for other units were missing bulbs 30 Days N
based on observation from the street. Provide light bulbs in
all light fixtures.
