The proposed work includes A proposed 120-square foot deck (10'x12') with a 12-square foot stairway (3'x4)is proposed off the back of the house for a total square footage of 132. The deck will be fastened to the house,as well as supported by three (3) 12"x 48" concrete footings on the western side. Footings will be installed into an existing patio and/or existing lawn. The proposed deck is approximately 74-feet from the BVW at its closest point. The area between the proposed work and the wedand slopes up and contains ledge,both of which serve as a barrier for any potential erosion. During the May 26,2021 public meeting, the North Andover Conservation Con-unission found this project to qualify as a Small Project Permit and voted unanimously to approve this project as proposed under Category"A"subject to compliance with the conditions listed bclow.The project is approved only as it-relates to the Conservation Commission and any and all other approvals should be obtained by the applicant/homeowner. All work shall conform to the following: RECORD DOCUMENTS: Small Project Filing Including: Application Checklist, Deck Specifications Updated Plan, Aerial Map with measurement to wetland The following conditions are hereby mandated: CONDITIONS: 1. Erosion controls shall be installed to the north of where the proposed work will occur and pre- construction inspection with the Conservation Department shall be scheduled. 3. Excavated soils from footing holes shall be taken off site. 4. Post-construction site visit to ensure compliance. 5. This permit shall expire one year ftom the date of issuance. Date of Meeting: El The delineated resource area boundaries have been confirmed by the Conservation Department as accurate. Plan Requirements(Section 4.4.3 13): Certified and/or professionally prepared plans are not required. However,the North Andover Conservation Commission or its designated agent shall have the final authority to determine whether the submitted plans/drawings are adequate and include all pertinent information for review. At a minimum,all plans submitted shall clearly and accurately show the present conditions and pjUosed work and depict the location of all wetland resource areas within 100'of the proposed work. All plans must be signed and dated by the person who prepared them, The Commission may also require the following information on the proposed plans in addition to the aforementioned criteria where applicable: ❑ Location of any subsurface sewage disposal systems. ❑ Locations of any drainage structures, ❑ Locations of any easements,including but not limited to,drainage and utility easements. ❑ Locations of any open space boundaries if(e.g. if the lot is affected by the 50' open space boundary as required in a Planned Residential Development(PRD)). ❑ Location and nature of all proposed alterations to the Buffer Zone Resource Area.
