Re: Subsurface Sewage Disposal System Plan for 249 Rea Street (Mai) 38, Lot 143)
Dear Mr and Mrs. Busanovich:
The proposed wastewater system design plan for the above site with a final revision date of May
24, 2021 has been approved.
The design plan has been approved for use in the construction of an upgrade on-site septic
system utilizing a Presby Enviro-Septic Leaching System under their Remedial Use approval.
The design reduces the required offset from the water table to the bottom of the soil absoprtin
system by 2 feet. This design plan approval is valid until June 8, 2023.
During this time, a licensed septic system installer must obtain a permit and complete this work,
and a Certificate of Compliance be endorsed by the installer, designer and the Town of North
Andover. In the event an imminent health problem, such as sewage backup into the dwelling is
occurring,the North Andover Board of Health may reduce the time period for which this plan is
valid.
This approval is also subject to the following conditions:
1. If site conditions are found in the field to be different from those indicated on the
design plan and/or soil evaluation,the originally issued Disposal System Construction
Permit is void, installation shall stop, and the applicant shall reapply for a new
Disposal Systems Construction Permit (310 CMR 15.020(1)).

2. The selected installer to build this system must have been certified for such work by
the product manufacturer and proof of such certification noted. Additionally, the
manufacturer in their letter to the engineer regarding this project dated March 26,
2021 have requested that only an installer who is"thoroughly familiar with the
Enviro-Septic Design and Installation manual requirements for Massachusetts" shall
be selected. A letter from the manufacturer with their concurrence that the selected
installer meets their stated requirement is to be provided.
3. A composite sample of the System Sand is to be secured and analyzed for
conformance with acceptable requirements for use with the technology proposed.
4. It is the responsibility of the applicant and/or the applicant's septic system designer,
septic system installer or other representative to ensure that all other state and
municipal requirements are met. These may include review by the Conservation
Commission, Zoning Board, Planning Board, Building Inspector, Plumbing Inspector
and/or Electrical Inspector. The issuance of a Disposal System Construction Permit
shall not construe and/or imply compliance with any of the aforementioned
requirements.
5. Prior to the issuance of the Certificate of Compliance the System Owner shall provide
a copy of the recorded and/or registered deed notice required by 310 CMR 15.278(10)
indicating the presence of a Presby Enviro-Septic Leaching System under their
Remedial Use approval.
