All structures were cleaned of debris, after adjusting frames and covers, orgrates, to finish grade, prior to paving in September2017. Silt sacks were placed and remained in place until directed to be removed. Silt Sacks were removed,from structures near Osgood Street(parking area at entrance to new administration building), on March 2nd 2018, at the direction of Conservation Commission. Upon removal of the silt sacks, structures were inspected and found to be clean and working as expected. Silt sacks at rear of the property remain in place at the request of Conservation Commission. Cutts, a duly Registered Professional Civil Engineer in the State of Massachusetts, hereby certify* that the work contemplated by the Notice of Intent and permitted by the Order of Conditions issued by the Town of North Andover Conservation Commission under MADEP FILE #242-1681 on July 15, 2016, and the work approved under Site Plan Review by the Town of North Andover Planning Board on July 5, 2016 has been completed in substantial compliance and within acceptable construction tolerances with the requirements of the Order of Conditions, the Site Plan Review Decision and the Approved Plans and will function as intended by the design. This change was approved by the DPW. • The water line in Wayne Street that was to connect to Osgood Street was terminated just beyond the new office building water connections due to the depth of the water main in Osgood Street. This change was approved by the DPW. • The pavement patches in Wayne Street have settled and will require additional pavement surfacing. • The existing tree located approximately 28 feet southeast of the proposed office building (toward Osgood Street) was damaged due to a construction incident and was removed. A new mulched plant bed with three shrubs has been planted in approximately the same location. • The configuration of the sidewalks, handicapped ramps and new catch basin at the entry driveway from Osgood Street was modified from the approved plans. The catch basin was moved northwesterly due to a conflict with existing uncharted utilities. The sidewalks leading to the proposed location of the handicapped ramps (near the street line of Osgood Street)were shortened and the handicapped ramps were moved to directly align with the Osgood Street sidewalk resulting in a more direct handicapped route and less impervious surface. This change was generally limited to the paved areas immediately surrounding the salt shed and did not result in any changes to drainage patterns or to the limits of work. • The CB that was converted to a DMH (adjacent to new double CB #5) has been covered by the crushed stone in the area. In our opinion none of the above items are significant to the intent of the project design and will not affect the project performance. Therefore, we believe that the work as completed is in substantial compliance and within acceptable construction tolerances with the requirements of the Order of Conditions, the Site Plan Review Decision and the Approved Plans and will function as intende sign.
