Re: Subsurface Sewage Disposal System Plan for 328 Campbell Road (Map 106D, Lot 24)
Dear Mr. Wear:
The proposed wastewater system design plan for the above site dated March 9, 2015.
Unfortunately,the plan cannot be approved until the following items are corrected. The specific
section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by this design
follows each item where applicable.
1. The proposed distance between the estimated seasonal high groundwater table and the
bottom of the soil absorption system is proposed as 3.0' on some sections of the design
plan and 3.5' on others and on the Application for Local Upgrade Approval. Clarify
2. Depict the water service line beyond the small section which is depicted and clarify if it is
a private well or a public water supply connection 310 CMR 15.220)
3. All piping is required to be PVC schedule 40,which conflicts with some plan notes that
specifies others (NA 3.2)
4. The full legal boundaries of the facility being served are not shown on the design plan.
(310 CMR 15.220(4)(a)).
5. The lot area needs to be indicated (NA 3.2)
6. Distances from lot lines and structure to the soil absorption system need to be indicated
(NA 3.2)
7. Indicate if the parcel is within the Lake Cochichewick watershed (NA 3.2)
8. As part of abandonment of the existing system indicate that the cesspool contents are to
be pumped by a licensed septage hauler(310 CMR 15.354 and NA 3.2)
9. Compliance with the wetlands setback is not maintained (NA 3.9)
10. The building sewer is to be noted to be laid on a compact and firm base and in a
continuous grade and in a straight line (310 CMR 15.222)

11. The septic tank is to have the tees located under the manholes and on the center line (310
CMR 15.227)
12. The design plan specifies use of an effluent filter but does not indicate the brand and
model, nor specify the required annual maintenance or cover to grade (310 CMR 15.227
and NA 3.2)
13. It does not appear 9" of cover is specified over the tank. Indicate a proposed spot
finished grade (310 CMR 15.228)
14. Specify the orifice size for the leaching pipes as 3/8"-5/8" diameter(310 CMR 15.251)
15. Clarify the cleanout specified in the Construction Key Notes as it is not apparent on the
site plan
16. The Site Plan does not have a scale associated with it
17. The barrier should be depicted on the Flow Profile
18. Provide a construction detail of the concrete slab that is begin requested beneath the
septic tank
19. Clarify Note 11 below the septic tank detail regarding the water tightness specification
which is to be achieved as it does not reference a typical standard used in North Andover
Although not a reason for disapproval, you may wish to consider the following:
A. Groundwater elevation might be lower than what is used as the basis for your calculations
B. The Construction Key Notes do not appear to be in the correct order. To avoid confusion
during the installation process please review and adjust as might be needed
C. It is possible the B soil does not need to be removed as indicated on the design plan
D. The location of the percolation test was not depicted on the site plan
