Hi John,
I wanted to reach out as we never received the 3 hard copies of the as-built plan for 740 Forest Street. We also never
received the signed installation certificate from both you (or the engineer from The Morin-Cameron Group) and the
installer.
Please submit 3 hard copies of the as-built plan and the signed certificate of installation as soon as possible. We did
receive the tank verification for the existing tank. I also CC'd the installer on this email as well.
Any questions, please let me know.

The Septic tank located at ?4L Forest Street worth Ar'�dv`✓cr in good condition with no
evidence of leakage. Tank size is 1,500 in volume (10'x5'x5'dp.)
It is 4' below grade with risers to grade,24'' Dia. Plastic domed Covers are screwed down for
safety.
Tank is a H2O (heavy duty) tank
Inlet baffle in good condition, outlet tee is new . Liquid was level with bottom of outlet invert.

The final construction inspection form is attached to this email.
The soil absorption system and distribution box construction was found to be acceptable and the installer was authorized
to backfill around that area.
Several issues were noted around the septic tank, as follows:
• The design plan calls for System Inspector to examine the existing tank to determine if it is acceptable to remain in
use or if it needs to be replaced. This was never completed and the installer just assumed the existing tank could be re-
used. I explained to the installer that a System Inspector would need to make that determination. He reports he will make
that happen.
• If the current tank is to remain in use there are several items in need of attention:
o A manhole to grade is needed over the inlet side of the tank because there are two pipes tied in
together outside the tank and this is a risk point for clogging
o The pipe exiting the tank did not appear to be a fully watertight connection with the tank and will need
to be secured with hydraulic cement
Additionally, the water line was reported to not run in the location shown on the design plan. I have let the engineer know
to depict the water line location correctly on the as-built and to depict the two building sewer pipes on the as-built plan, if
they are not already shown.
I know Steve is out for the week so I have instructed the installer to keep me apprised.

Re: Subsurface Sewage Disposal System Plan —740 Forest Street (Map 105D, Lot 34)
Dear Brian:
We are in receipt of your review of the septic design for the above referenced property dated
January 21, 2021. Please find enclosed the following:
• 3 sets of the revised Sanitary Disposal System Repair Plans
• Application for Local Upgrade Approval (Form 9A)
On behalf of Stuart Thompson, The Morin-Cameron Group, Inc. has prepared the following
responses to your comments. For your convenience the response comments are numbered in the
same order and correlate to the numbered comments in your letter:
1. A local upgrade approval application (Form 9A) has been included for a reduction in
setback between the proposed effluent line and the existing water service crossing. It
should be noted that the existing effluent line crosses the water service. The proposed
effluent line shall be sleeved for a distance of 10' on each side of this crossing. A note for
the local upgrade has been added to Sheet 1 of 2 and the profile has been revised to
reflect the water service and sleeve on Sheet 2 of 2.
2. The plan view (Sheet 1 of 2) has been revised to show benchmarks within 50-75 feet of
the proposed facility.
3. The existing septic tank is located 13' from the foundation wall and is proposed to be re-
used. If replacement is required, the tank will be replaced in the same location at existing
inverts. A dimension depicting the distance from foundation wall to tank has been added
to the plan view (Sheet 1 of 2).
4. Notations stating "Outlet distribution lines shall be level for a minimum of the first two
feet of their length" and "All outlets of the distribution box shall be equal to each other
and located at least two inches below the invert elevation of the inlet" have been added
to the notes on the distribution box detail (Sheet 2 of 2).

5. The distribution box detail has been revised to show "9" min., 36" max." cover (Sheet 2 of
2).
6. The soil evaluator has discussed the submitted soil testing forms with Mill River
Consulting. It has been agreed upon that the submitted forms are sufficient and DEP
forms 11 and 12 will not be required.
7. The leach bed detail has been revised to show 12" min., 36" max." cover (Sheet 2 of 2).
8. The perforated pipe shown on the leach bed detail has been revised to show "3/8" - 5/8"
ORIFICE (TYP)". (Sheet 2 of 2).

The revised plan meets the standards in Title 5 and the local regulations and we would recommend approval. Note there
is a Local Upgrade Approval request accompanying this submission for reduced setback from sewer line to water line. A
draft approval letter is attached should you wish to use it.
