SITE CONDITIONS
❑ Contractor reports any changes to design plan

Comments: 5/24/21 — D. Ottenheimer: Laundry waste pipe was intercepted and
tied into the main building sewer pipe just outside the septic tank. Contractor
reports water service line does not ru where shown on the design plan and was
not encountered during installation. Contractor was informed of plan requirement
to have existing septic tank examined by a System Inspector to determine if it
can be re-used or not.
SEPTIC TANK

Comments: S. Casey — 4.20.2021 LxW of bottom of bed @ 53x24, needs to be
56x26. Rocks/tree branches/limbs in bottom of bed. Hole needs to be fixed for
inspection. John Morin verified enginee had been out.
S. Casey — 04.23.2021 — 56 '12' x 27', rocks pulled, Will on site. Sand coming
Monday.

The final construction inspection form is attached to this email.
The soil absorption system and distribution box construction was found to be acceptable and the installer was authorized
to backfill around that area.
Several issues were noted around the septic tank, as follows:
• The design plan calls for System Inspector to examine the existing tank to determine if it is acceptable to remain in
use or if it needs to be replaced. This was never completed and the installer just assumed the existing tank could be re-
used. I explained to the installer that a System Inspector would need to make that determination. He reports he will make
that happen.
• If the current tank is to remain in use there are several items in need of attention:
o A manhole to grade is needed over the inlet side of the tank because there are two pipes tied in
together outside the tank and this is a risk point for clogging
o The pipe exiting the tank did not appear to be a fully watertight connection with the tank and will need
to be secured with hydraulic cement
Additionally, the water line was reported to not run in the location shown on the design plan. I have let the engineer know
to depict the water line location correctly on the as-built and to depict the two building sewer pipes on the as-built plan, if
they are not already shown.
I know Steve is out for the week so I have instructed the installer to keep me apprised.

evidence of leakage. Tank size is 1,500 in volume (10'x5'x5'dp.)
It is 4' below grade with risers to grade,24'' Dia. Plastic domed Covers are screwed down for
safety.
Tank is a H2O (heavy duty) tank
Inlet baffle in good condition, outlet tee is new . Liquid was level with bottom of outlet invert.
