Re: Subsurface Sewage Disposal System Plan for 693 Johnson Street(Man 38,Lot 75)
Dear Mr.Nemchenok:
The proposed wastewater system design plan for the above site dated July 18,2021 has been
reviewed. Unfortunately,the plan cannot be approved until the following items are corrected.
The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by
this design follows each item where applicable.
1. A Local Upgrade Approval for only having one test pit in the soil absorption system area
was requested. This is not prudent as the size of the soil absorption system and the
variable soils noted on other parts of the site warrant further analysis of existing site
conditions(15.102).
2. It appears that the bottom of the septic tank may be below the ESHWT. Please determine
the ESHWT elevation in the proposed tank location and provide buoyancy calculations if
required(15.221).
3. Depict the ditance from the soil absorption system and the drain line and catch basin, and
indicate if these components intercept the water table or not(15.211 and NA 3.9)
4. Provide a benchmark within 50'-75'oposed onsite wastewater system(15.220)
5. Visually depict the proposed tank to have tees located beneath the tank openings so it
coincides with the written note provided(15.227)
6. Provide a percolation test within the proposed soil absorption system(15.104)
7. Confirm the distance from the proposed soil absorption system to the wetlands and
indicate who determined the resource area boundary(15.211 and NA 3.9)
8. The leach field end section on sheet 2 references a barrier on the site pan which is not
identified. Clarify this

9. T-1 observed water depth is incorrectly noted. Clarify this
