Work involves: After-the Fact ermittin for work involving re- furbishing existing patio surface and installation of a open pergola over patio footprint. The patio has been in its existing location at least since 2005 based on aerial photography and is 42'6" to the wetland at its closest point. No other work is permitted or allowed under this Determination of Applicability, as the entire property contains wetland or associated 100- foot buffer zone and is subject to regulation under the State Wetlands Protection Act and North Andover Wetlands Protection Bylaw. The request must be made bycertified mail or hand delivery kJ the Department, with the appropriate filing fee and Fee Transmittal Form (see Request for Departmental Action Fee Transmittal Form) as provided in 310 CMR 10.03(7)within ten business days from the date of issuance of this Determination.
