Ottenheimer DATE OF FINAL GRADE INSPECTION: SITE CONDITIONS ❑ Contractor reports any changes to design plan ❑ Existing septic tank properly abandoned ❑ Internal plumbing all to one building sewer ❑ Topography not appreciably altered Comments: 10-6-21 — Scope of work for this installer was to remove the existing soil absorption system and sand fill and re-built it and also make sure the pump configuration is appropriate. All work seems correct. D. Ottenheimer SEPTIC TANK ❑ Building sewer in continuous grade, on compacted firm base ❑ Cleanouts per plan ❑ Bottom of tank hole has 6" stone base ❑ Weep hole plugged ❑ 1500 gallon tank has been installed H-10 loading ❑ Monolithic tank construction ❑ Water tightness of tank has been achieved by visual testing ❑ Inlet tee installed, centered under access port ❑ Outlet tee installed, centered under access port (gas baffle/effluent filter) ❑ inch cover to within 6" of finish grade installed over one access port ❑ Hydraulic cement around inlet & outlet Comments: 10/6/21 — Riser installed to grade over septic tank effluent filter. Installer reports floats were properly secured to float mast. D. New bottom of bed is only to ensure all sand that did not meet Title 5 requirements was removed, bed bottom was empty, and new sand was acceptable. 9.23.2021 All Sand removed, new load ok, Installer to dewater hole before additional load of sand installed 10/6/21 — Soil absorption materials and elevations appear correct and appropriate.
