Fax (978) 686-5100 www,aridoverconsultants.com July 12, 2017 North Andover Planning Board c/o Jean Enright, Planning Director Town Offices 120 Main Street North Andover, MA 01845 RE: Watershed Special Permit 1661 Great Pond Road Dear Members of the Board: As required by the Watershed Special Permit Notice of Decision, Special Conditions 46, our office has completed an as-built survey of the property for the project referenced above. Andover Consultants, Inc. was not retained to conduct inspections during construction so the attached plan depicts site features that were visible, or accessible from the surface, at the time of the survey only. Based upon a comparison of the original design plan titled "Site Plan Great Pond Road Road", dated March 28, 2013 and revised June 3, 2013,prepared for Technical Training Foundation Trust with the attached plan titled "As-Built Plan 1661 Great Pond Road" dated July 6, 2017, prepared for TKZ, LLC and a field inspection conducted on June 19, 2017, it is my opinion that the project was completed in reasonable compliance with the approved plan. The following minor differences are noted for your reference: • The as-built driveway is more irregularly shaped and generally further west than the one depicted on the approved plan, the as-built sidewalk is smaller and starts further west than that depicted on the plan, but the dwelling foundation footprints are the same. As a result, the as built impervious area is approximately 60± square feet, only, more than the proposed impervious area. « The stone swale is also more irregularly shaped and wider than depicted on the approved plan. • A bulkhead was constructed in the back of the dwelling which was not depicted on the approved plan. • There is a rear stair constructed over a crushed stone area which was not depicted on the approved plan. • There is 12-foot wide± crushed stone strip at the back of the drive that was not depicted on the approved plan ® There is a deck on the back of the dwelling that was not depicted on the approved plan.
