Andover Consultants, Inc. was not retained to conduct inspections during construction so the attached plan depicts site features that were visible, or accessible from the surface, at the time of the survey only. Based upon a comparison of the original design plan titled"Site Plan 1665 Great Pond Road Road", dated March 28, 2017 and revised through May 25, 2017,prepared for TKZ LLC and the attached plan titled"As-Built Plan 1665 Great Pond Road" dated July 25,2018,prepared for TKZ, LLC and a field inspection conducted on July 19, 2018, it is my opinion that the project was completed in substantial compliance with the approved plan. It should be noted that some site features depicted on the final approved plan which is referenced in the "Notice of Decision Watershed District Special Permit Modification"dated June 8th, 2017, were not built. These site features primarily include a f1,150 sf proposed patio and a 24'xl4' proposed pool. What was actually built more closely resembles the plan approved in the original"Notice of Decision Watershed Special Permit" dated April 5, 2016 which includes a deck and walkway. The following minor differences are noted for your reference: • As described above, the pool and patio were not built, and instead a deck with hot tub was built per original approved plan. • The stone filter strip heading towards Great Pond Road was built wider than what was shown on the approved plans, allowing for additional stormwater capture and recharge. • The generator was moved from the side of the house to the rear along the existing retaining wall. On behalf of TKZ, LLC, we respectfully request that the Board release the performance bond being held for this project. v-ilPl2-2da:al.send2UlPA1?6 W'S4as[ItiCo Pimce_IGfi3Ae Civil Engineers o Land Surveyors • Laird Planners X andover J consultants Inc. The site plan should identify the location of the two proposed evergreens of minimum 10' height. 3. The site plan has been revised to remove the existing masonry walls at the driveway entrance. TEC recommends that the Applicant remove the existing masonry wall up to the limits of the property line. The Applicant could remove the remainder of the masonry wall if requested by the neighboring property owner. 4. A 2'x10" stone filter strip has been added as an outlet from the crushed stone infiltration bed. This filter strip provides additional treatment and infiltration and directs stormwater overflow away from the existing drop inlet structure and into the Swale on the north side of Great Pond Road. Please feel free to give me a call this week if you have any questions! T0652.07 Re: Peer Review #1 Watershed Special Permit Application 1665 Great Pond Road, North Andover, MA Dear Ms. Oldham: On behalf of the Town of North Andover, TEC, Inc. conducted a technical review of the April 11, 2017 Special Permit Application for the project modification located at 1665 Great Pond Road. TKZ, LLC ("Petitioner") submitted the following documents, which TEC reviewed for conformance with Section 4.136"Watershed Protection Division"of the Town of North Andover Zoning Bylaws: • Site Plan, 1665 Great Pond Road, North Andover, MA, prepared by Andover Consultants, Inc., dated March 28, 2017; • Special Permit— Watershed Permit Application, 1665 Great Pond Road, North Andover, MA, prepared by TKZ, LLC, dated April 11, 2017; • Description of Proposed Project, 1665 Great Pond Road, North Andover, MA; Additionally, TEC reviewed the submitted documents in comparison to the Notice of Decision for a previously approved Special Permit for the Watershed Protection District of North Andover dated April 5, 2016. Upon review of the documents and plans, TEC has compiled the following comments for the Board's consideration: 1. The proposed pool is located within the 250-foot Non-Disturbance Zone. The proposed pool is a use allowable by Special Permit in accordance with Section 4.136.3.c.ii(3) of the Town of North Andover Zoning Bylaws only after a variance has been granted by the Zoning Board of Appeals (ZBA). The Petitioner has indicated that a variance is being requested from the ZBA. 2. The Petitioner is proposing to install a new trench drain and 20'x20' crushed stone infiltration bed within the 100'wetland buffer and the entire property is located with the MassDEP Zone A which will require permitting with the Town of North Andover Conservation Commission, The Petitioner has indicated that a Request for Determination of Applicability is being filed. ,/3. The Site Plan should be revised to label the 100'wetland buffer. N 4. The crushed stone infiltration bed and patio filter strip are proposed within the 325-foot Non- Discharge zone. These sub-surface discharges are allowable by Special Permit in accordance Plan I Permit I Design I Construct Ms. Rebecca Oldham 1665 Great Pond Road — Peer Review April 20, 2017 Page 2 of 3 with Section 4.136.b.ii(1). Available GIS information indicates that natural soils onsite consist of fine sandy loam, which is acceptable for infiltration, however, no field collected soil data has been submitted with the application. %/111 5. Section 4.136.4.c.ii(2) of the Town of North Andover Zoning Bylaws states that the edge of all wetland resource areas are to be confirmed by the Conservation Commission if the edge of wetland resource area is within four hundred (400) feet of any proposed activity. It should be confirmed that the wetland boundary shown on the plan has been confirmed by the Conservation Commission. ,/6. The Petitioner should revise the plans to delineate the proposed limits of work in accordance with Section 4.136.4.c.ii(6) of the Town of North Andover Zoning Bylaws. 1/7. The Application requires a written certification by a Registered Professional Engineer, or other scientist educated in and possessing extensive experience in the science of hydrology and hydrogeology, stating that there will not be any significant degradation of the quality of the or quantity of water in or entering Lake Cochichewick in accordance with Section 4.136.4.c.iii. of the Town of North Andover Zoning Bylaws. 8. There are two existing pine trees in the vicinity of the proposed crushed stone infiltration bed that are not shown on the Site Plan. The trees should be added to the Plan and if necessary, the infiltration bed should be redesigned to retain the two trees. Approximately 1,900 square feet of impervious driveway flows to the trench drain and infiltration bed. The infiltration bed is proposed in a —13% slope. The bottom of the infiltration bed should be constructed flat (or sloped at 2% max) to promote infiltration and increase recharge volume. If constructed in this manner, the infiltration bed will be sized to handle a 1" rainfall (160 cubic feet) meeting industry standards. The crushed stone should be placed to match the existing topography. 10.TEC recommends that an Operation and Maintenance Plan be submitted to maintain the dw proposed trench drain. At a minimum, the trench drain should be cleaned twice per year, focused in the early spring and late fall. 11.TEC recommends that silt socks be installed to the east and north around the proposed pool and patio area. 12.The proposed patio is 870 square feet larger than the approved patio in the Notice of Decision dated April 5, 2016. A patio filter strip of crushed stone is proposed around the edge of the patio to mitigate the impervious area. TEC recommends that permeable pavers be used as a patio surface to further reduce runoff volume and increase groundwater recharge. ,v,"i3.TEC recommends that the use of lawn care products on the site be limited to organic or slow- release low nitrogen and low phosphorus fertilizers and the use of coal tar-based pavement sealants is prohibited. ]":Al'Or)!32\1'0652,07\Doc,,3\1,et,tei,s\TO652,07,,.Watei,sl,ied Review 1665 Great Pond.docx MEC Ms. Rebecca Oldham 1665 Great Pond Road — Peer Review April 20, 2017 Page 3 of 3 14.The Petitioner should identify and employ best management practices to protect the watershed from contamination by swimming pool chemicals. Draindown and Maintenance Prior to seasonal closure or draining of the pool for maintenance, chlorine levels will be reduced to undetectable levels of<0.1 mg/l, either by: 1) eliminating chlorine usage for approximately ten days to allow chlorine levels to dissipate, or 2) utilizing sodium thiosulphate or other pool dechlorination chemicals approved by DEP for use in watershed areas. Water will only be discharged over the extended lawn areas to the east of the patio and residence, which will allow further filtration and infiltration of the water. Overflow Protection Whenever a severe or 100-year storm event is predicted, the pool water level will be drained, pursuant to the above methods, to a level to permit 7 inches of rain to be collected in the pool Infiltration Trench Maintenance Infiltration trenches will be inspected at the seasonal opening and closing of the pool, and after major storm events per above. Debris will be removed, and any silted or clogged stone will be replaced as needed to maintain the design performance of the trenches.
