Attached please find the Installation certification. Tom will need to sign this form and have it scanned back
to me, and then I can send it over to Jack to sign (or vice versa).
Jack, once you have them finalized please submit 3 hard copies of the As-built plans to us for review. If
either of you have any questions, please let me know.

The construction inspection completed on Friday afternoon at this address identified some issues which need attention.
1. The septic tank/pump tank in the front yard was not watertight and had clean water infiltration, likely due to the very
high water table in the yard and inadequate craftsmanship by the installer to prevent this
2. The force main in the rear yard septic tank was plumbed into the outlet end of the septic tank not the inlet end of the
septic tank. There also was no Tee to divert the incoming water into the middle layer of the tank
3. The pump in the final pump chamber was not operational
Report is attached. Let me know any questions.

Report of construction inspection at 432 Salem Street is attached.
A few small items are noted:
• Pump and alarm not wired so could not be checked
• D-box needs hydraulic cement and need to see flow to it once pump is live
• Riser over effluent filter needs to be at finished grade
Otherwise all looked good. They are going to start backfilling the leach field but keep the d-box open for further
examination.

We have completed our review of the design plan with the new pump and pump tank proposed for the front of the
dwelling to bring the sewage into the rear where it ties into the main septic system. We believe this design complies with
Title 5 and have crafted the attached approval letter for your use.

We reviewed the plan for 432 Salem Street which now has a pump unit proposed to be located in the front yard.
Unfortunately there is some unclear information and likely some other issues which are not in compliance with the
requirements in Title 5 for pumping to a septic tank. I sent the email below along to the design engineer.
Just a heads up that it might end up being that they wish to seek a variance from Title 5. 1 will keep you posted if that
ends up being the case.

The plan that was produced for 432 Salem Street which includes the pump tank in the front yard dated 8/31/21 was
reviewed.
A few items:
• Unfortunately it does not seem as if the requirements for pumping to a septic tank in 310 CMR 15.229 are complied
with. This is a poorly worded section of the code but basically part 1 needs to be complied with if<25% of the flow is
being pumped as sewage, and parts 1 and 2 need to be complied with if more than 25% of the flow is being pumped as
sewage. I am operating under the assumption that both part 1 and part 2 of that section apply since it seems the majority
of the sewage from the house exist towards the front. If that is an incorrect assumption please do let me know.
• There are three different pump models specified. Clarify which one is intended and then provide specifications for
that pump (can be a separate cut sheet if desired). The pump selected will also determine if 229(1) (a)or(b)applies.
• Comply with the tank standards at 15.229 (2) (b) [or seek a variance]
• Provide power or emergency storage as indicated at 229 (2)(c)

I had to add a 500 gallon pump chamber in the front yard of this property since two bathrooms
from this house discharge to a mystery tank out front that Arco found. This tank was not shown on
any record plans but the owners told me they believed a tank was in the front yard (I noted this on
the approved septic design plan previously submitted).
So...Arco uncovered the front tank and since this house is on a slab the ability to re-route the
internal plumbing to the rear is not an option. I have updated the approved septic plan to show a
proposed 500 gallon conc. pump chamber, 2 inch pvc force main (to connect to the newly installed
1500 gallon conc. septic tank in the rear yard, provided a pump chamber detail with relevant
elevations, provided a system profile on Sheet 1, added info. on the pump curve on Sheet 2, and
added a note for the existing front tank to be pumped and decommissioned.
I have attached the plans and I can upload to the permitting site...I wanted to see how many plan
copies you will need and what the fee is (if any) for the review..
As you know the approved system to the rear has been installed by Arco...so this is the last step to
complete this project.
Liz...you can use this plan for pricing of the change...
