HomeMy WebLinkAboutConstruction Notes - Consultant Review - 432 SALEM STREET 11/17/2021 North Andover Health Department Community and Economic Development Division ONSITE WASTEWATER SYSTEM CONSTRUCTION NOTES LOCATION INFORMATION ADDRESS: 432 Salem Street MAP: 037.B LOT: 0052 INSTALLER: Tom Sawyer, Arco Excavators 603-895-0111 DESIGNER: Jack Sullivan 781-854-8644 PLAN DATE: 07.09.2021 BOH APPROVAL DATE ON PLAN: 09.29.2021 INSPECTIONS TANK INSPECTION: 08.17.2021 DATE OF BED BOTTOM INSPECTION: 08.19.2021 DATE OF FINAL CONSTRUCTION INSPECTION: 8/31/21, 10/29/21, 11/2/21 DATE OF FINAL GRADE INSPECTION: 11/2/2021 SITE CONDITIONS Contractor reports any changes to design plan Existing septic tank properly abandoned, if present Internal plumbing all to one building sewer Topography not appreciably altered Comments: 8/31/21 – As noted in previous communication, there is a building sewer in the front of the dwelling as well as one in the rear. The house configuration does not allow the installer to merge the two building sewer pipes into one. A new design plan for a pump system to bring the sewage to the rear will be provided. D. Ottenheimer 10/29/21 – Inspection completed to examine the septic tank and pump tank installed in the front yard as well as the issues which remained unresolved in rear yard from previous inspection. D. Ottenheimer 11/2/21 – Construction inspection to examine unresolved issues SEPTIC TANK – FRONT YARD Building sewer in continuous grade, on compacted firm base Cleanouts per plan Bottom of tank hole has 6” stone base Tank size and loading per plan Monolithic tank construction If monolithic, water tightness of tank has been achieved by visual testing If 2-piece, water tightness of tank has been achieved by water / vacuum test (circle one) Inlet tee installed, centered under access port Outlet tee installed, centered under access port effluent filter installed (circle one) If effluent filter, riser to finished grade over filter Riser with cover to within 6” of finish grade installed over one access port Manhole and riser minimum 24 inch diameter Pipe penetrations appear watertight using rubber boots 10/29/21 – Tank in front yard examined. Found to not be watertight. Likely at pipe penetration on outlet, also possibly at risers. Very high water table showed infiltration. D. Ottenheimer 11/2/21 – Hydraulic cement placed around riser/tank interface and riser/lid interface and better securing mechanism provided at pipe penetration. Watertightness has been demonstrated satisfactorily. D. D. Ottenheimer 11/2/21 – Force main moved to inlet side of septic tank and tee installed. Tee not under opening in tank and is to be adjusted. Also watertightness of former opening on outlet side of the tank needs to be demonstrated. Do. Ottenheimer PUMP CHAMBER Bottom of tank hole has 6" stone base Weep hole plugged 1000 gallon Pump Chamber installed H-10 loading Monolithic tank construction Inlet tee installed, centered under access port Pump(s) installed on stable base Alarm float working Pump On/Off floats working Drain hole in pressure line __” cover at final grade installed over pump access port Water tightness of tank has been achieved by Visual or watertightness testing Neoprene boots around inlet & outlet Comments: 8/31/21 – Pump and alarms not yet installed. D. Ottenheimer 10/29/21 – Pump was unable to be made operational. D. Ottenheimer 11/2/21 – Pump and alarm demonstrated to properly function. D.
