Hope you are well.......
We are finalizing the septic as-built for the above referenced property. Find attached a DRAFT copy of the as-built plan.
Back in Feb. 2020 James Melvin PE, one of the engineers in our Haverhill office, prepared an interim septic as-built for
this property that showed all the system components and elevations. It did not include the foundation since it was not in.
Therefore it did not include ties from the foundation or the invert at the foundation. The DRAFT as-built plan attached
shows all this information. I believe the reason for the interim as-built was so that the site contractor could back fill the
septic system before the foundation was installed.
One of our engineers showed me the checklist you sent over for the septic as-built. A lot of that information required per
the checklist we have not shown on septic as-builts in North Andover for a while. Could you please review the attached
DRAFT plan and verify if it has sufficient information that it would be approved.
Another issue we have is James Melvin, PE, performed the system inspections and he no longer works for our company
so we will not be able to have him sign the Certificate of Compliance form regarding the inspections.

Hi John,
Thanks for getting this over to me. I will review it today and let you know if I have any questions or believe there is
information that needs to be added. As for the installation certification, was James Melvin the only one who conducted the
inspections to verify it was installed according to the approved plans? We can certainly discuss this more
moving forward.

Understood. I think the notation on your as-built plan about that is fine. Assuming that the TOF = 127.96 noted is your
benchmark, we only need a couple things added to the as-built. We need the location of the control panel, and if you
could also add the location of the propane tanks & line to the house that would be great. Once you add those, and add
the stamp&signature you will be all set. I will add these emails to the file as well since they note that James Melvin is not
an employee anymore.

We were waiting for Jim to finish everything before we finalized the septic as-built. We will modify the as-built to include
all the information required including the elevation benchmark, ties to the existing dwelling and the invert at foundation.
We will get that done and submitted as soon as we can.

Hi John,
I hope all is well.Jim Heider(Owner and Installer at 562 Boxford Street in North Andover)reached out to try and tie this
project up as it has been ongoing for approximately 3 years while they built the home.
Looking back in our file and emails with Mill River, it appears the major item that was never addressed was the
benchmark. The initial benchmark was removed/altered and a new one was set but never confirmed by the designer
(early in the project, this was Christian &Sergi, however the As-built was completed by James Melvin of the Morin-
Cameron Group). We need a letter from the Morin-Cameron group authorizing the new benchmark used for the basis of
construction and noting that it is correct.
Additionally,we received the as-built from the Morin-Cameron group and I had a couple outstanding items (which was
expected as the house was not constructed yet). If we can update the as-builts based on the checklist I attached and
resubmit 3 hard copies with corrections/updates that would be great. I also attached the installation certification which
was signed by Jim Heider back in 2018. We would need this to be signed and dated as well.
As this project has been ongoing for a while, please don't hesitate to let me know if you have any questions. I have CC'd
Brian LaGrasse, our Director, as I will be out of the office tomorrow 8/31 until 9/9. Worse case scenario, we can speak
more once I am back in the office on the 9th.

I tried to do the installer a favor at this site when he called me about meeting him last week. I told him that we had not yet
heard from the Town, and it was a bit out of the ordinary but since I was going to be nearby I'd see what I could do, but
that I needed him to have the designer follow the standard protocols and contact your office to say everything was
acceptable to perform a final inspection.
I have not yet heard from you that the designer has done that. I called the installer today and he was not sure either.
Once you get the ok to release me to do the final construction inspection then please let me know, and please send along
the started construction inspection form (if there is one).

Also,just a reminder that we need a letter from the designer authorizing the new benchmark that was used as the basis
for construction and that it is correct. I spoke with the installer today and he was not sure if that had been completed.
Lastly that I will be out of town Thursday through next week. I doubt much will come up in the dead of winter but I also
did not want you to be surprised if you called or emailed and I was not around.
