I hired Northeast Classic Engineering on 9/22/20 to install a septic system at 125 Boston St.Things did not go smooth
through the project. When the tank was installed,Jim wanted his second payment.At that time, he informed me that
there was$10,100.00 in extras,which I strongly disagreed with. He pulled off the job without the leach field being
connected. He said he would not come back until I paid the extra money.About a week later there was a sewerage smell
in the back yard. I called Dan Giard, and he came out that afternoon and pumped the tank. He said it was full and
couldn't believe the sewage didn't come into the house. Being held hostage by Jim, I paid$5,000.00 in extras with the
agreement the job would be completed with no additional costs.
The work on site was completed as far as I could tell. I called Amy Maxner to see if there was anything else that was
needed.She told me as built plans and inspection reports needed to be submitted. I contacted Jim and he said it was
not his job to submit plans. He needed to put a final invoice together and I had to pay that before he would give me the
plans, and I would have to submit them. When the invoice came,there was a final payment due as per contract, but
again there were extra charges.We agreed to meet at the town hall on 10/7/21 to square this away. He was late, so I
went in and talked with board of health. I can't remember her name, but she was very helpful and pulled out a form that
Jim needed to complete and sign. When Jim arrived, he would not come in, and texted me to meet outside. I had a
check ready for him for the final payment per contract plus recording fees, construction permit fee, and abutters
notification cost. He would not accept the check. He wanted an additional $1,650.00 to release the plans to me. I
wouldn't pay the additional money,so he left with the plans and said he would put a lien on the house.
I have contract, invoices, and emails if needed.
Sorry for the length of this but wanted you to get a picture of what's been going on.

In Regard to use of Cultec C-4 Chambers
Innovative/alternative Technology
I Herby Certify and attest as the present owner of the property listed above that:
• I have received a copy of the Title 5 I\A technology approval and the
owner's manual for the proposed Cultecc-4 chambers to be installed.
• That a garbage disposal will not be installed.
• That the SAS will be replaced, modified or other actions taken as required
by the Board of health or DEP if the system is determined to be failing to
protect human health and safety and the environment.
• That the system will only be installed by an installer licensed by the Board
of Health who has received appropriate training by Cultec.
• That prior to the issuance of a certificate of compliance the designer and
installer will certify that the system has been installed in accordance with
all necessary requirements.
• That If I find the SAS has ponded 3" or more I will measure the ponding
level within 30 days and report to the Board of Health the ponding level if it
remains at 3" or more. This measurement is made via the observation port
shown on the plans.
• That when a feasible sanitary connection becomes available I will connect
to the facility within 60 Days.
• I will notify any new owners that there is an alternative SAS in use at the
above referenced propert .

We have reviewed the revised plan for this location dated August 20, 2020 and find it to be complete.
Please note there is one local upgrade request with this design to reduce the distance from the bottom of the soil
absorption system to the water table, and one variance to the North Andover regulations to be less than the required
distance to wetlands from the soil absorption system.

Jim,
Have the loam and seed been placed for 125 Boston or 544 Johnson?We need to conduct the final grade inspections
after the area has been graded, loomed and seeded. As a reminder, I will need someone to accompany me to 544
Johnson to open the tank and ensure the inlet and outlet tees are in compliance.

Pending weather, I could do the inspections at 12:30 or 3pm. If you do not wish to be present for 125 Boston Street that is
not an issue as I don't need to view the tees at that location. Let me know if one of those times works better. If it is raining,
we can reschedule 544 Johnson Street.

plastic chambers proposed to be used, there is no justification provided for the Local Upgrade Approval request, and
there are some missing pieces of information on this design plan.

Jim,
The tank hole at 125 Boston Street was acceptable today. I noted 6 or more inches of stone in the hole where the tank is
to be placed (as the hole was oversized slightly).
As the licensed installer(and designer/engineer)you must be onsite for the construction of this system. Please ensure
you are present for the bottom of the bed, and have verified it is to plan before I come out for that inspection. If you have
any questions, please let me know.
