T0758.01 Re: Land Disturbance Permit— Mill Road &Johnson Road (Lots 1A, 1B 2A & 2B) Civil Engineering Peer Review Dear Ms. Enright: On behalf of the Town of North Andover, TEC, Inc. reviewed documents as part of the civil engineering peer review for the proposed project to be located at Mill Road &Johnson Road (Lots 1A, 1B 2A & 2B). Sullivan Engineering Group, LLC ("Applicant'% submitted the following documents, which TEC reviewed for conformance with the General Bylaws of the Town of North Andover and the North Andover Stormwater Management and Erosion Control Regulations: • Land Disturbance Permit Application — prepared by TKZ, LLC, not dated; • Plan of Subsurface Sewage Disposal System, prepared by Merrimack Engineering Services, dated September, 10, 2016, revised June 6, 2017; • Site Construction Details, prepared by Sullivan Engineering Group, LLC, dated December 13, 2017; • Stormwater Management Plan — prepared by Sullivan Engineering Group, LLC, dated November 16, 2017; • Construction Storm Water Pollution Prevention Plan — prepared by Sullivan Engineering Group, LLC, dated November 16, 2017; and • Operation and Maintenance Plan — prepared by Sullivan Engineering Group, LLC, dated November 16, 2017. Upon review of the documents and plans, TEC has compiled the following comments for the Board's consideration: 1. Section 7.1.h — it appears that some of the submitted plans are illegible. The Applicant should revise plans to clearly demonstrate all existing and proposed line work, dimensions, texts, etc. TEC recommends the Applicant include a legend demonstrating existing and proposed lines/symbols. 2. Section 7.1.n —The Applicant has not provided existing and proposed vegetation on the site. 3. Section 7.1.p—The Site Plans do not clearly depict the pre and post construction watersheds, subwatersheds and stormwater flow paths. The Applicant should provide separate plans demonstrating the pre and post construction watersheds and stormwater flow paths in accordance with this Section. Section 7.1,r—The Applicant should perform a test pit in each of the proposed drainage infiltration systems to ensure adequate separation to estimated seasonal high groundwater. 5. Section 7.1A—The Applicant should provide further detail for the proposed drainage infiltration systems. The Applicant should show roof drains connecting into the drainage infiltration systems and provide details. Drawings should be provided for the infiltration systems consistent with Section 7.1.t. 6. Section 7.1 —A locus map at a scale of 1"-800; should be provided. Furthermore, names of abutters should be provided on the site plans. 7. Section 7.2.B.d —The Applicant should demonstrate the locations of the designated control points utilized in the HydroCAD Analysis. 8. Section 8.0 —The Applicant should provide an Erosion and Sediment Control Plan and include all required details described in Section 8 of the North Andover Stormwater Management and Erosion Control Regulations. 9. Proposed soil stockpiles should be relocated so they are not directly on top of the proposed infiltration systems. 10. It appears that the Applicant is proposing to infiltrate stormwater within 50'of the proposed septic tank/leaching field systems; the Applicant should revise the plans to ensure a fifty (50') foot minimum separation between infiltration and leaching systems to be in compliance with the Massachusetts Stormwater Handbook. 11.The project is proposing septic systems that will require review and approval by the Town of North Andover Board of Health. 12.The Applicant should include a section in the stormwater report to document how each proposed lot within the project complies with the 10 standards in the Massachusetts Stormwater Handbook. Areas within the limit of work line would have existing trees removed, but is willing to walk the site in the spring with the Town Planner to determine specimen trees that could be saved within the work area.The proposed landscaping for these four lots will be conventional single family landscaping methods consisting of lawn areas and mulch beds/plantings around the houses. TEC. TEC agrees with the Applicant If amenable to the Board, a spring site walk to determine trees to be saved could be considered as a condition of approval. I Section 7.1.p—The Site Plans do not clearly depict the pre and post construction watersheds, subwatersheds and stormwater flow paths. The Applicant should provide separate plans demonstrating the pre and post construction watersheds and stormwater flow paths in accordance with this Section. Applicant Response: Revised Predevelopment and Post development drainage plans has been prepared to better demonstrate the watershed areas,subcatchment areas, Stormwater flow paths (time of concentration flow lines), and proposed drainage mitigation (for the Post development scenario). The full drainage Study involved the area of disturbance on each lot. Design points are clearly labeled. TEC• Comment Addressed. 4. Section 7.1.r—The Applicant should perform a test pit in each of the proposed drainage infiltration systems to ensure adequate separation to estimated seasonal high groundwater. Applicant Response: Test pits were performed on all 4 properties by others and witnessed by the North Andover Board of Health for the approved septic system locations.The proposed drainage infiltration fields are all approximately SO feet from soil testing location and the infiltration rates used for the drainage system is consistent with the percolation rates established. Estimated seasonal groundwater was established on each lot (by others) and these values were used in the drainage design to ensure a minimum 2 foot vertical separation between the water table and the bottom of the drainage system. TEC•According to Volume 3, Chapter I of the Massachusetts Storm water Handbook, a Title 5 percolation test is not an acceptable test to determine the design infiltration rate(Saturated hydraulic conduch vity)for a stormwater Best Management Practice. TEC recommends that the Applicant revise the stormwater calculations using the infiltration rates listed in Table 2.3.3 of Volume 3 of the Storm water Handbook based on the textural analysis that was performed for the Soil Absorption System. 5. Section 7.1.t—The Applicant should provide further detail for the proposed drainage infiltration systems. The Applicant should show roof drains connecting into the drainage infiltration systems and provide details. Drawings should be provided for the infiltration systems consistent with Section 7.1.t. Applicant Response: Although the final house plans have not been developed for these four lots, roof drain connections have now been shown on the Site Plans. Also, a detail has been provided for the roof drain connections showing an emergency overflow to a splash pad for storms that may exceed the 100 year storm event. TEC., The infiltration system on Lot JA is proposed with a "top of unit"elevation of 225.9. The proposed grade at the location of the system is approximately 224 The infiltration system would protrude above glade by about 2 feet, The Plans should be revised to account for the system. 6. Section 7.1 —A locus map at a scale of 1"-800; should be provided. Furthermore, names of abutters should be provided on the site plans. Applicant Response: The locus map at a 1"=800' has been attached to his letter. Names of the abutters are shown on the Site Plans. TEC.• Comment addressed 7. Section 7.2.B.d —The Applicant should demonstrate the locations of the designated control points utilized in the HydroCAD Analysis. Applicant Response: All design control points have been labeled on the Pre & Post Development stormwater plans. TEC., Comment addressed. 8. Section 8.0 —The Applicant should provide an Erosion and Sediment Control Plan and include all required details described in Section 8 of the North Andover Stormwater Management and Erosion Control Regulations. Applicant Response: The Erosion and Sediment Control Plans have been revised to show all critical design features including soil stockpile areas, construction entrance,erosion control barriers, drainage features, and limit of work. The detail should include a 2' minimum separation to groundwater. Where a groundwater separation of 2' is not possible,an impermeable liner should be installed beneath the system. If this comment is agreeable to the Board, I think it would acceptable to make this a condition of Approval.
