Dear Property Owner:
The North Andover Board of Health has completed the review of the septic system design pens
for the above referenced property, submitted on your behalf by Pennoni Associates Inc,, dated
July 2 , 2010 and last revised and submitted on February 27, 2012. This plan has been approved.
The design has been approved for use in the alteration and additional constr{uetion to an existing
onsite septic system for a design flow of 2145 gallons per clay based on a combination of square
footage and uses. In accordance with state regulation this plan approval is goad for 3 years,
During this time, a licensed septic system installer must obtain a permit and complete this work,
and a Certificate of Compliance be endorsed by the installer, designer and the Town of North
Andover.
This approval is subject to the following conditions.
Prior to a Disposal Wor-ks Construction Permit is issued a construction sequence schedule
must be submitted to and approved by the Health Department. The construction sequence
shall provide details at nnininnrm as to;
a) how the new building will be constructed while the existing gas station remains open for
business.
b) when the septic construction is proposed to occur within this schedule
c) when the drainage structures are proposed to be built
d) when the existing pump tank pumps will be turned off; leaving the alarms in service
2) Pt#iott to per-ruit issuance; The issuance of the disposal works construction permit is
contingent upon the receipt of a foundation as-built of the dwelling. The as-built must be in a
scale of 1" = 20`. It must show the elevations of the slab and all building sewer inverts at
trrrnrmurn.

circumstances detailed in the letter written September 7, 2010 by the designing engineer, Bern

Osgood, Jr. P.E. As this project is being treated "more like a municipal sewer system rather
than a septic system" in regards to the Pipe issues, it is being required that the septic installer
not only be licensed by the N. Andover Health Department for septic installation, the
contractor also roust show proficiency in drain laying by providing proof of a drain layers
license or equivalent verifiable experience upon application.
4) Additional Inspections; In additiotr to the usual inspections, the engineer is required to
personally view the bottom of bed to ensure and verify that the existing sand is clean and
acceptable for the system.
5) If site conditions are found in the field to be different from those indicated on the design plan
and/or soil evaluation, the originally issued Disposal System Construction Permit is void,
installation shall stop, and the applicant sliall reapply for a new Disposal Systems
Construction Permit.
G) It is the responsibility of the applicant and/or the applicatrt's septic system designee, septic
system installer or other representative to ensure that all other state and municipal
requirements are met. These may include review by the Conservation Commission, Zoning
Board, Planning Board, Building Inspector, Plumbing Inspector and/or Electrical Inspector.
The issuance of a Disposal System Construction Permit shall not construe or imply
compliance with any of the aforementioned requirement.

Dear Susan:
Enclosed are revised plans for the above reference property which Incorporate the labeling of
the 0" sewer lines as requested.
In regards to the issue as to why an 8" sewer line was used in lieu of a typical 4" building sewer
Is concerned, In many Instances a large residential site, usually one having multiple units, or a
commercial site has a sewer system which is more complicated than a site with a single building
sewer which connects to a septic tank. In these Instances It Is more appropriate to design a
system between the building and the septic tank in a manner which is more like a municipal
sewer system rather than a septic system. Because of the number of building sewer outlets and
to make maintenance and Inspection of the building sewer lines easier a manhole to manhole
system was used on this design. In addition, the pipe size of the manhole to manhole system
was enlarged to B" so that a slope of 0.005 could be used in lieu of the 2% normally provided in
a building sewer. This was required to keep cover over the pipe as well as to keep the depth of
the septic tank from exceeding the 3 foot maximum.

Dear Ms. Hughes:
On April 18, 2011, Wetlands Preservation, Inc. (WPI) conducted an inspection of the property referenced
above. This report is presented in accordance with Conditions #50 and#51 of the Order of Conditions
(OOC) issued on July 27, 2010 and having Massachusetts Department of Environmental Protection
(DEP) File#242-1478. The following observations were made with regard to reporting requirements as
specified in the OOC.
Construction activity on the site includes installation of fill material. Soil lift compaction reports should be
provided to the North Andover Conservation Commission, Engineering Department and WPI. All silt fence
and hay bale lines are intact. Site grading has controlled site nrnoff rather than interim erosion controls between
lifts. WPI observed minimal evidence of soil erosion and no sediment impacts to down gradient resource areas.
Each day that fill material is placed; site grading is maintained in a manor such that storm runoff is diverted
away from the side slopes at the end of the day. Clean crushed stone of sufficient size 6"+_ interspersed with
smaller stone should be on hand for creating temporary water bars to slow storm runoff and directing flows
away from the side slopes. The construction entrance was clean at the time of the inspection and the roadway
had been cleaned of sediment.
