Could not access basement to examine plumbing but it is a newer house and likely only has one building sewer pipe existing. D. Ottenheimer SEPTIC TANK ® Building sewer in continuous grade, on compacted firm base ® Cleanouts per plan ® Bottom of tank hole has 6" stone base ® Tank size and loading per plan ® Monolithic tank construction } ® If monolithic, water tightness of tank has been achieved by visual testing ® Inlet tee installed, centered under access port ® Outlet tee installed, centered under access port ® Effluent filter installed ❑ If effluent filter, riser to finished grade over filter ❑ Riser with cover to within 6" of finish grade installed over one access port ❑ Manhole and riser minimum inch diameter ® Pipe penetrations appear watertight using rubber boots / hydraulic cement (circle one) Comments: 12/20 — 6" stone noted across tank & pump chamber hole. Existing tank still hooked up until new ones arrive. 12/29/21 — Building sewer has bend in it so a cleanout to grade was installed. Pipe penetration into inlet side of septic tank is at a severe angle however it appears watertight and was not requested to be fixed. Effluent filter selected is not one proposed by designer. Manhole to finished grade over effluent filter to be installed. D. S. Casey 12/29/21 — Contractor appears to have built trenches 2.5.'-3.0' wide instead of 2.0' as indicated on design plan thus the spacing between them is reduced. Not a critical error which would trigger the removal and re-construction of the trenches but note worthy enough to be documented herein. D. Mill River noted that the filter observed during the final inspection at 37 Scott Circle was not a Zabel A1800 filter but it was a Polylok-brand effluent filter instead.
