Re: Subsurface Sewage Disposal System Plan for 199 Old Cart Way (Map 107B,Lot 108)
Dear Mr.Nemchenok:
The proposed wastewater system design plan for the above site dated February 4, 2022 has been
reviewed. Unfortunately, the plan cannot be approved until the following items are corrected.
The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by
this design follows each item where applicable.
1. A Local Upgrade Approval for only having one test pit in the soil absorption system area
should be requested (15.102)
2. Visually depict the proposed tank to have tees located beneath the tank openings so it
coincides with the written note provided(15.227)
3. Note the location of existing impervious areas (15.220)
4. Depict the location of the existing septic tank, if known,and provide instructions for
abandonment(15.352 and NA 3.2)
5. Indicate the need for magnetic marking tape over the system components (15.221)
6. Consider using trenches or explain why they cannot be used in this instance ((15.240)
7. Indicate the orifice size of the perforated pipe to be used(15.251)
8. Indicate in the relevant details that the leach stone and the pea stone are to be double-
washed(15.247)

While not a reason for disapproval please consider:
A. Providing a note to accompany the visual depiction of the stone beneath the tank detail to
identify what is to occur
B. MassDEP has issued a new Form 11 which should be used for future soil evaluation
reports
C. Remove the notation about soil removal at the T-2 soil log to avoid confusion
