On December 15th, 2021 you were issued a twenty-four(24)hour notice that enforcement measures would be taken if the violations noted were not corrected. After reaching out numerous times via phone and email to you, you have not responded to our efforts to contact you nor have you corrected the remaining violations listed in the letters sent to your property. Therefore,the North Andover Health Department will be filing in Housing Court against you as the property owner unless the following items are addressed in the following time frames: • Bedroom at the top of the stairs must have window repaired or replaced to function correctly within 3 days per 105 CMR 410.500. • Second bedroom(blue room)has bowing ceiling and damage or defects present in multiple areas on ceiling, walls and the floor. The window in this room is also in disrepair. The window in this room must be repaired to function correctly within 3 days per 105 CMR 410.500. Repair all ceiling, floors and walls to be free of defects within 5 days per 105 CMR 410.500. • Bedroom next to bathroom missing doorknob,provide working doorknob within 3 days per 105 CMR 410.500. This bedroom's windows are also in disrepair, as one falls into the room at random and two have ripped gaskets and don't remain open when unlocked. Repair all windows to function properly within 3 days per 105 CMR 410.500. • Second floor railing is loose and not sturdy. Repair railing within 3 days per 105 CMR 410.500. • The screen door on the rear entrance must be repaired or replaced within 5 days per 105 CMR 410.500. • Window screens throughout the back-porch area must be repaired or replaced to be free of defect within 5 days per 105 CMR 410.551. • Light fixture in the back-porch area and the switch operating it must be repaired or replaced within 5 days per 105 CMR 410.253(A). • Kitchen ceiling tiles that were damaged and had evidence of water damage must be repaired or replaced, and the source of the water damage must be investigated and repaired within 5 days per 105 CMR 410.351(A). • Outlet on the counter to the right of the sink must be repaired or replaced within 5 days per 105 CMR 410.351(A). • Dining room next to kitchen had multiple damaged ceiling tiles that must be repaired or replaced within 5 days per 105 CMR 410.500. • Door to the basement must be repaired to remain closed once shut within 5 days per 105 CMR 410.500. • Broken window pane in living room must be repaired or replaced within 5 days per 105 CMR 410.501(A)(1). • Interior front door handle that is loose must be repaired or replaced within 5 days per 105 CMR 410.500. • Bedroom at top of stairs has evidence of water damage and cracking on ceiling. Investigate source of water damage and repair all damage within 5 days per 105 CMR 410.351(A). • Second floor hallway has a hole in the wall. Repair hole within 5 days per 105 CMR 410.500. • Bathroom window is missing a handle and does not open. The ceiling in this room is chipped and has peeling paint across it. Repair bathroom window to function properly and defects in ceiling within 5 days per 105 CMR 410.500. • Closet near bathroom has light fixture that does not work inside. Repair light fixture within 5 days per 105 CMR 410.253(A). Failure to repair all violations within the outlined timeframes above will result in the Health Department filing against you in Housing Court, as well as potential fines and other enforcement procedures.
