We have completed our review of the revised plan at this address. There is one small typographic error that is not
consequential so not worth having the plan revised.
This design is predicated on their request for a Local Upgrade Approval. Should you choose to grant that approval then
the attached letter can be issued.

Please provide the DEP approval letter and whatever other supporting documents might go along with the particular type
of disposal system being proposed in this instance. Sometimes DEP has kind of blanket approvals for groups of different
types of systems (such as an STU)and then often also has an approval letter for a particular technology.
We need to review the usage conditions in those documents against what was proposed and if we need to spend time to
find these documents and figure out which is the correct one you are proposing to implement it might take a while.

The use of this technology also needs to comply with DEP's Standard Conditions for General and Remedial Use Approval
of Alternative Soil Absorption Systems according to their approval letter. If I could trouble you for that as well it would be

We pushed aside other matters to take a look at the revised submission which came in at the end of last week for 544
Johnson Street since the project is in mid-construction. If any of the comments below do not make sense or are not clear
I apologize in advance but we wanted to get this looked at as quickly as possible.
Our review found the following items that appear to need to be resolved or explained. These are organized by the origin
or location of the material:
DEP Standard Conditions (this allows for the usage of this type of disposal system with treatment and provides the
conditions for their usage)
o A deed notice will need to be provided and recorded, please provide a draft
o The designer needs to certify the plans to be in compliance with the Standard Conditions and the approval
o The system owner needs to acknowledge ownership responsibilities (section 18 d)
o The plans need to show the maximum area needed for a conventional system
o The plans need to show a best feasible replacement system
o The designer needs to submit proof of any training by the manufacturer which is required prior to design (if any)
Remedial Use Approval (this provides specific conditions for the GeoMat Leaching System being proposed)
o Pressure distribution is required when GeoMat is installed directly into native soil
o Inspection access is required at the end of each run of pipe
GeoMat Leaching System design manual
o When installed in native soils GeoMat shall be separated by 2 times the width of the GeoMat used. This is not
provided on the design plan.
o No 5 foot overdig is demonstrated on the plan and is indicated to not be required in the design manual. This is not
indicated in either of the above documents issued by DEP. Please obtain clarification or provide for an overdig.

o Remove reference to Cultec design certification as that is not the soil absorption system being proposed.
o Clarify why there is the"Maximum Feasible" note requesting less than 4' of natural soil below the soil absorption
system as the design plan shows test pits with adequate soil depth.
o With a breakout elevation of 93.32 it does not appear the required 15' horizontal distance of that elevation is met.
o Explain why the breakout elevation should not be at the top of the GeoMat at elevation 93.57 or adjust the design plan
to reflect this.
o General note 12 references a reserve area though none is apparent on the design plan.
Lastly, please note the following future requirements so they can be planned accordingly:
o The installer needs to demonstrate that he is certified or has received appropriate training by the manufacturer.
o Prior to issuance of a Certificate of Compliance the installer and designer must certify compliance with the Standard
Conditions, and the deed notice needs to be recorded at the Registry and a copy provided to the Health Department.

[note that I have removed 2 people on this thread that are with the Town as they do not need the details of our technical
conversation]
We have a fundamental disagreement on a few items which need to be addressed. I think the core of it is that we are
reviewing your submissions primarily in light of the 2 approvals issued by DEP which allows this system to be used in
Massachusetts, and you are completing your design plan based on the manual produced by the manufacturer. I could be
wrong, but I am not sure they are in sync. Our lens for a review is Title 5 and whatever technology approval letters exist
and not so much a manual produced by a manufacturer so you will need to make sure this design is in compliance with
the Standards Conditions and the GeoMat approval.
To that end what I see as large unresolved questions are:
A. The GeoMat has different design standards when placed in native soil and in sand fill. One uses gravity distribution
and the other uses pressure distribution. The sand fill meeting C-33 specifications immediately below and above the
GeoMat product is an integral part of their system and is part of what the product relies on for treatment to occur. That
layer, in our mind, is not"sand fill"as it is a required part of their system. In the case of this address, it appears the
GeoMat product(C-33 sand and the distribution mat and piping) is going to be installed in native soil and thus would need
to be designed in accordance with the native soil requirements in the product approval. You claim this is not needed.
Provide documentation to demonstrate this as our read of the approvals does not concur, or else re-design to provide
compliance.
B. When used in native soil the approvals indicate the need to have the lines of GeoMat separated by at least twice
their width. You claim this is not needed. Provide documentation to demonstrate this as our read of the approvals does
not concur, or else re-design to provide compliance.
C. The approvals do not appear to waive the requirement for a 5' overdig around the system. You claim this is not
needed. Provide documentation to demonstrate this as our read of the approvals does not concur, or else re-design to
provide compliance. That said, as you know, a 5'overdig is only required when the breakout elevation is above the
existing grade so at this particular address it is possible that no overdig is required or perhaps only part of the system
needs an overdig.
D. Inspection ports are required at the end of each run per the DEP approval. You claim this is not needed. Provide
documentation to demonstrate this as our read of the approvals does not concur or else re-design to provide compliance.
E. Provide documentation from the manufacturer that no designer or installer training is required as indicated below, or
else provide proof of completion of required training.
If I am off the mark with anything please do let me know. As a courtesy I am attaching the Standard Conditions and the
GeoMat approval which are being used as the basis for our review.

Thank you again for handling this Dan. I was, once the plan is approved and all details are finalized, going to reject the
old permit(citing ledge encountered/site conditions)and issue a new DWC permit for the new system, a new approval
letter for the new plans and obtain 3 hard copies like we usually do. Once Jim squares everything away with you I will

Thanks for sending along the revised plan, draft deed notice and critically the most recent approval letter for the type of
disposal system with treatment you are proposing to use at 644 Johnson Street. Many of our unanswered questions
have been resolved.
Understanding you were burning the midnight oil in some of the plan edits we did notice a few items in need of attention
before moving forward. It is important to have the plan correctly noted for future reference and to avoid problems during
installation so please review the following and make adjustments as might be needed:
1. The site plan indicates a different building sewer elevation from the Design Elevations table
2. The 16' x 25' rectangle for the proposed soil absorption system you are using along with 1' of sand around it should
be labeled on the site plan
3. The top and bottom elevations of the proposed barrier should be indicated
4. There is no Local Upgrade Approval needed to meet the breakout requirements in Title 5 as you are proposing the
use of a barrier. Remove that notation to avoid confusion
5. You are requesting a Local Upgrade Approval for less than 4'of naturally occurring soil beneath the soil absorption
system. Such an approval cannot be granted pursuant to Title 5. Remove that notation to avoid confusion. As a side
note, the approval for the disposal system with treatment which you are proposing to use allows there to be less than 4'of
naturally occurring soil beneath the leaching facility
6. The profile should depict a riser to grade over the effluent filter in the tank, and risers as needed over the d-box to
bring the cover within 9" of finished grade
7. The distance between the tank and distribution box in the profile is not correct
8. The citation for note#23 in the profile is not correct and should reference note#21 or some other note as you think
appropriate
9. The maximum area needed for a conventional system and the best feasible replacement system on the site plan are
not acceptable in the location proposed due to the presence of shallow depth to ledge

2. The 16'x 25' rectangle for the proposed soil absorption system you are using along with 1' of sand around it
should be labeled on the site plan
3. The top and bottom elevations of the proposed barrier should be indicated
4. There is no Local Upgrade Approval needed to meet the breakout requirements in Title 5 as you are proposing the
use of a barrier. Remove that notation to avoid confusion
5.You are requesting a Local Upgrade Approval for less than 4' of naturally occurring soil beneath the soil absorption
system. Such an approval cannot be granted pursuant to Title 5. Remove that notation to avoid confusion. As a
side note, the approval for the disposal system with treatment which you are proposing to use allows there to be
less than 4' of naturally occurring soil beneath the leaching facility
6. The profile should depict a riser to grade over the effluent filter in the tank, and risers as needed over the d-box to
bring the cover within 9" of finished grade
7. The distance between the tank and distribution box in the profile is not correct
8. The citation for note#23 in the profile is not correct and should reference note#21 or some other note as you think
appropriate
9. The maximum area needed for a conventional system and the best feasible replacement system on the site plan
are not acceptable in the location proposed due to the presence of shallow depth to ledge

three red circles is attached.
1. 1 was envisioning the risers on tank and d-box on the scaled profile but I only called it the "profile" in my email to
you so that might have been a source of confusion. I see where you call for them, but please also depict them on the
scaled profile for clarity
2. The theoretical conventional system area should be located where there are test pits which demonstrate the
suitability of that area. No test pits are present where it is currently depicted. Please move to an acceptable location.
3. The note on the site plan which is circled does not seem in consort with the other elevations specified for the
building sewer pipe. Please clarify.

We have completed our review of the most recent design plan iteration completed for 544 Johnson Street. It is dated July
6, 2021. We believe it is complete and ready for endorsement and issuance of a DSCP.
As such we have prepared a draft approval letter for your consideration. Note that items 2, 3 and 4 are unique to the type
of disposal system being used. Item 2 and 3 are required in the approval letter for the type of system being used and I
chose to repeat them here for emphasis, you can delete if you think it is not needed. Item 4 is something that I wrote to
bring clarity to the special sand fill that is an integral part of the GeoMat Leaching System, however feel free to edit if that
reimbursement process is not something that is feasible.

Never a dull moment.....
It seems like the sand complies with Title 5 standards but is too fine to be considered C-33 sand. If this is the
material he used to bring the system up to final grade then it seems like it can remain, but the material immediately
below, above and to the sides of the GeoMat will need to be removed and replaced with sand that meets the C-33
specification.

Please find attached the sieve analysis results from the sample you and I took at 544 Johnson Street. Unfortunately it did
not meet the C-33 Specifications called for under the geomat noted on the plan.
The sand immediately below, above and to the sides of the geomat system will need to be removed and replaced with
sand meeting the specifications of C-33 sand. Since it does meet the Title 5 requirements, anything under the system
past the 6 inches of C-33 can be kept. If you have any questions, please do not hesitate to contact me.

Attached please find the construction inspection report for the project located at 544 Johnson Street.
You will note that a deficiency was identified during the installation of the soil absorption system in that one arm of the
piping was higher in elevation than the distribution box outlet. This was corrected and then re-examined.
Also, the contractor did not have tools to open the manhole covers so I was never able to examine the tank for proper
tees. He did eventually have workers come over to fix the problem with the soil absorption system and it was likely they
had the proper tools however with the issues identified and being corrected at the soil absorption system it frankly fell
through the cracks to check the tees on the tank. I have no reason to think the contractor did not build these, but perhaps
if the final grade inspection has not been completed yet you might wish to have him open one or all of the manholes to
confirm.

I wanted to follow up before the final grade inspection was ready. At this inspection I will need to view the inlet and outlet
tees in the tank as they were not able to be inspected during the final construction inspection. Please give me at least a
