Dear Mr. Chairman:
Enclosed please find three copies of Site Plan Application stamped received by the
Town Clerk of North Andover. Also -enclosed are ten copies of the site plan, abutters list,
envelopes, filing fee in the amount of$1,433.76 and review fee of$3,000.00.
The proponei it proposes to construct a 14,768 square foot CVS Retail Pharmacy
with site improvements including 81 parking spaces oil a 2.86+ acre project site located on
the north side of the intersection of Salem Turnpike (Massachusetts Route 114) and Peters
Street in North Andover, Massachusetts. Site frontage is provided to the southeast along
the Salem Turnpike and to southwest along Peters Street. The site is zoned General
Business. There are no wetlands resource areas located on or near the site.

Preliminary plan, modification of a Preliminary Plan, Modification of a Definitive
Plan, or Modification of a Special Permit shall require the following initial Project
Review Fee:
