Two proposed"monument"signs are shown on the plans at the drive entrances,although no details are provided. Are these directional in nature? Will they have external illumination? Dimensional data? The Applicant should clarify what these are,and if any freestanding site signs or details are proposed. At this point no main site sign appears to be proposed. (Not Addressed. At the very least,the directional signs should be labeled and detailed appropriately,and a note placed on the plan that no freestanding sign is proposed.) Responw: VVe are indicating one 'directional'sigii at the entrance to CVS off Peters Street. A detail is provided oil the detail sheet, C-12. All styles 4es and materials will be similar to the building, which 0 the planning board has approved the concept of. CVS through the building inspector, as indicated ill the signage bylaw. 2. The proposed wall signage needs specifications for colors,area,confirmation on projection dimensional limits,and materials. (Not Addressed. Usually the Planning Board requires this information for their review,although the Building Commissioner would have final review for the sign permit issuance. See comment below for example.) Therefore,the maximum number of signs allowed is one primary and one secondary, which are both assumed to be two wall signs'?(thus any freestanding signs need to be addressed vis-a-vis the ByLaw);(c) supplementary accessory signs as they relate to the Bylaw; (d) percentage calculations as required in 6.6.D.1,which should be provided on the plans. (Not Addressed) Response: The building inspector will be supplied ii,-ith till signage hijbI-mation. No signage will be installed without approval fromthe building inspector. Section 8.1 Parking 2. The Peters Street driveway is not provided with an adequate leveling platform,which should be evaluated due to the long 7%grade off that drive. (Please address what appears to be a 20-foot leveling area measured from the stop bar that exceeds 2% and then drops to over 7%--this will be the only means of site egress for all other travel directions other than the one-way out onto Route 114 North,and at a drive intersection that will require some delay waits queuing out of the drive. Also,please review the sidewalk ramps shown at the crosswalk). Response: A leveling area is provided at the Peters Street entrance, as indicated on the reNsed Site Plan set. A sit cient landing area has been provided at this location. ffi Section 8.3.5 Supplementary Regulations-Site Plan Review—Information Required e.iv) The proposed construction easement for the wall needs to be verified by the abutter via an easement deed reference note on the plans. (Not Addressed. At a minimum,the board needs to be assured that the abutting land owner approves this.) Response: The applicant is currently engaged in discussions with the Board oj'Directors ofthe First United Methodist Church at this time. A letterfi-oin the FUMC will be provided to the Planning Board stating that the Church will approve of the temporary constritefioii easement. e.xvii) Please address possible conflicts between the wall-pak building light mounting height(z-value on detail table)and where those would be located and shown on the building elevations. Please also clarify pole details, including pole heights. (Not Addressed. Are the wall paks mounted on the roof,or are the z-values incorrect?) Response: The wall pack lighting heights have been changed to reflect the actual height of the lighting fixtures, which do not exceed the building height. The application has beenfiled u,iih,the Massachusetts Highway Department. Utility Plan &Details 1. As previously suggested,SMH 2 Rim will be raised up out of the pavement byabout 1/2 foot,and SMH-I may be slightly low—these issues could be avoided if the grading information was shown on the sewer design,from a Q/A standpoint. (Not Addressed) Response: All SARI rims have been mdiustedtoproperly reflect the correct elevations. 2. To provide improved visual screening and safety signage placement,consideration should be made to extend the landscape island near DMH-A5 to the north. This could be done and still provide compactor and building access,and would tremendously improve aesthetics at that site entrance. You may also need to look at the SU-Vehicle Turning Template. (Not Addressed) Response: The island has been extended 4' to provide more screening and traffic sa ty.for the compactor Oil area. Re: CVS North Andover Plan Revisions The following is summary of the minor drawing changes to the approved CVS site plans,for the proposed CVS at the corner of Route 114 and Peters Street: 1. MHD drainage connection-MHD required that the discharge from the site pass through a biofiltration swale,to provide additional pretreatment before entering the MHD drainage system. A Swale is now provided on the adjacent Eagle-Tribune property. The owner of the Eagle Tribune has agreed to provide a drainage easement for the improvements. 2. An additional sewer manholes and sewer line were added to the Peters Street side of the building,so that CVS would not have to run the sewer pipe under the building. Three additional manholes and associated sewer pipe have been added. If you have any questions,please let me know. This certification is based on my review of the"As-Built Survey",dated February 7,2005 by VHB and visual observations made on the site.Please note that the connection to the Route 114 drainage system has been completed,disregard notations on the As-Built survey which indicate that this connection has not been made. TEC's Contract with.PSRT was a time-based Contract from June 28, 2004 thru.December 31, 2004. TEC has been informed that PSRT has sold the property to your firm. As such, PSRT is no longer directly involved in this project and is not extending our current Contract as they consider our services complete.
