Kaminski and Associates , Inc. , attached. No equipment , trucks , etc. shall occupy any part of the travelled tray except between the hours of 8:00 A.M. and 4: 30 P.M. from Monday through Friday. Weekend work is not permitted under the terns of this permit. It shall be the responsibility of the Grantee to contact the District Highway Engi- neer regarding the field location of any underground traffic control devices oa this Project. .,aid driveway shall be surfaced with Bituminous Concrete, Type I and shall he - comprised of a 3" Class 1 Bituminous Concrete Base Course and two ( ) M" courses of Class I Bituminous Concrete Pavement for a total depth of 6" with a foundation of at least 6" of corepacted gravel . The finished surface shall butt into and not overlap the existing highway grade at thE road edge. All work shall be dome in accordance with the Ccmmopwealth of Massachuset-ts Department of Public Wofts Stand- ard Specifications for Highways and Bridges . The driveway shall be so gradod that no water shall enter the layout nor pond or collect thereon, including the roadway surface. All disturbed areas-, such as sidewalks , curbing, shoulder, gr�:ss plats, shall be restored to their original conditions . The curb corners or radii may ho painted at the time of installation. Said curb shall be Painted yellow only. The curb shall be placed in conjunction witlh or immediately following the completion of the driveway surfacing. The Grantee shall erect standard traffic regulatory warning signs according to the Stand- ards and Specificaliorrs of the Department. Said plan shall be signed and stamped by a registered professional engineer. The Depart- ment shall not accept this job as being complete until this plan is received. Said plan shall also note any change in elevations which night have been undertaken during the construction of this driveway. T The Grantee shall indemnify and save barmless the Commonwealth and its Department of ubiic :works against all suits, claims of liability of every name and naeure arising at any time o%lt of or in consequences of the acts of the Grantee and in the perfomance of the work covered by this permit and/or failura to comply with the terms and conditions of this permit whether by itself or its employment of subcontractors. This permit is subject to the procurement of the necessary Town Grant . If it becomes necessary the Departmient shall place an Inspector on this project and said Inspector will be- paid for under a reimbursable by the GRANTEE Two sway traffic shall be maintained at all nimes during this project . At arse tie during tlse operation of this project it becomes necessary for the Grantee to detour traffic it shall he his responsibilit7 to notify the District Highwav Engineer in writing seventy-two (72) hours in advance of this detour. The Grantee shall supply the District Highway Engineer with a plan showing the intended route of this devour along with the appropriata signing and the rrismber of Police officers to be assigned to this detour. The Graritee shall also state the length of time said detour will be in existence and he shall have concurrence from the area Police Chief regarding this detour. A-11 said detours are subject to review and concu rr ence by the District Traffic Engineer. If it becomes necessary to remove and reset any higlxway bounds then the Grantee shall_ hire a registered professional_ land surveyor to perform this work. it shall be the responsibility of this land surveyor to suit to this office a statement in writing and a plan contaJ-ning his stamp and signature showing that said work has been performed.
