Water tightness of tank has been achieved by
visual testing

Hydraulic cement around inlet & outlet
Comments: Septic tank is existing and to remain
4-28-22 – Contractor was not sure if there was an effluent filter on outlet side of
tank. Contractor did not have riser to grade over outlet side of tank. Contractor
reports he made a visual assessment and believes the tank, risers and
