A- SHORT TERM NLUN't'f'NAN ]PROGRAM DURMG CONSTRUCTION
1. A.Ri1r,inspection shalt be performed by a RE_ during tilt consttuctiwi of the grading,
reptication, reatorati<on and drainage facilities, and tko-5e improvements shown on the plan
whi0i will have an inapea on the wetland s at least once every week during the apphcable
construction. Insprxtions shall not be rr-quiredi during periods of inamivity to these
fui;ities_ A repon shall he generated and forwarded to the North Andover Conservation
Commission after each inspectiotn_
2. Care shall be taken at all times to control Prosiun and sedimQnt movement by compaction
of distudwd extas and the use of hay bale/silt fences as shown on the plan. Other than

5. Upon scheduling of and then successful earablishment of the wetlurzd replacement and
land restoration axeas an iuspeaion shall he perfotmed with an agtm of the Forth
Andover Conservation Commission and a report indicating that the construction and
ewaWishmerit of these arm are in sobstantiai conformance to the plans and Order of
Conditions shall be presented to the North Andover onsvrvaticn Commission.
6_ Partial Certificates of Compliance may be granted by the Forth Andover Conservation
Commission during construction of the project upon a wri tern request for each i&rrtifrecl
unit or gbemc.
7_ A fimal Certificate of Compliance may be granted by the North Artdovtr Conservation
Commission which shell b-t considered the commencement of the maintenance ptogfan_

LONG TERM MAINTENANCE PROGRAM
1_ On or about every May l and November i of each year a site inVection shal i be:
performed by a ragistered professjonal engineer(P.E)of the Commonwealth of
Massachusetts to report an the status of the "nage features and impact to wetland
areas} if any. The inspector YAU schedule the inspection with the responsible parties
49 hours prior to the inspection so arrangements cart be made to access all rnaraholes,
including if necessary, roping off of parking spares to assure manhole access. This
report shalI include the name, address and telephone number of the owlw of the
drainago facilities, the names, addr s and telephone numbers of parties responsible
for the:operation and maintenance ofthc; drainasc facnUti,,-a, and ain "dine of
�rertti irtxrng Rands in the line itrm of the budget for drainage miiintmianec. The report
shall also identify the current staUt8 of the drainage facility. The inspection shall
include a visual inspection ofeach drainage structure including the catch basins,
manholes, subsurface detention facilities, Stormceptor Unit, buffer planting, concrete
outlet structami, BhV's and any other suueture or oortdition relating to the long term
operation of the drainage facilities_ The report sW1 oudine any observed pollution, if
other than nDrxmal sand and Oil ranoff from driveway and parking areas-
2- On or about October 15 of each year a Landscape orntractor small complete,the
cleaning out of any debris in the non-catchbasin drainage structures or drainage pipes
and 5hal3 prepare a short written report of the work perfbrmed to be submitted to the
inspector(PE) for inclusion in his report. He will also inspect the buffer planting
area and replant any dead vegetation. He�halI report she replanting to the
Conservation Agent.
3. Once every year from: the date of commencement, on or about September 1, the catch
basirws and Stormceptor Unit shalI be thomughly cleaned by mechanical and/or manual
means to remove any debris and settleracrtt.
4. Once every year from the date of commencement, usually in ihv spring, a Landscape
ootractorshall regrade, reloam and reseed or stabilize any areas which have efoded
or developed wdiment or which have developed conditions not shown onthe plans,
written report of such activities shall be presented to the inspector for itxclusion in his
report_

5. If the xnspuptor reports that work needs to be done to maintain any faculty then the
work must be cc inpieted within tine next 12 week8, unless otherwi sic spm icd, the
Condition of which shall be r poned in the inspector's next report-
6- Snow, afte€ plowing, which is stockpiled, shall be removed from she site within 24
hours of plowing- Sand ortly and no ale-icing cvrnpounds or chemicals shall be used
for de-icing-
7, A]I chuck va Ives(S" backwater #lush valve) associated with the subsurface drainage
facilities shall be inspected and maintained three(3) times per year (April 1, JuIy 1,
and November 1),
