with all other applicable federal, state, or local statutes, ordinances, bylaws, or regulaflons.

6- This Order may be extended by the hssulng authority for one or rriore periods of up to three
years each upon application to the issuing authority at least 30 days,prior to the expiration
date of the Order. An Order of Conditions for au`feat Project may be extended for one
additional year only upon written application by the applicant, subject to tho provisions of 310
CMR 110.05(1 1)(f).
6. If tlfls Order constitutes an Arnenided Order of Conditions, this Amended Order of
Conditions does not extend the issuance date of the original Ftnal Order of Conditions,and
the Order will expire on unless extended in writing by the Department,

1T Prior to the start of work, and if the project involves work,adjacent to a Bordering Vegetated
Welland, the bo,ufldary of thie wet.land in the, vicinity of the proposed work area shaH, be
marked by wood'enstakes or flagging. Once in, place, the wetland: baundary markers shall
Imo maintained until a Certificate of Cornpliance has, been jSSUed by the Conservation
coni'mission,
'18, All,sedimentation barriers shall be maintained in good repair until all diStUrbed areas have
been fully stablilized with vegetation or other means. At no firn[e shall sedirnents, be,
deposited iin, a wetland or water body. During construction:,, the applicant or his1herdesignee
shall inspect the erosion,controls on a dafly basis and shall rernove a=mRflated sedfinents
as needed:, The applicant shall iminediately control any erosion problbrns that occur apt the
site and shall also irnmedlately notify the Conservation Coimrnission:, which reserves the
right to rOqLdre additionall erosion and/or daniage prevention controls uit may deern
nocossary. Sedi4nentation barriers shall serve as,the limit of work unless another firrift of

(including,all planting plans) approved by the iSSding authority, and have teen inspected to
enstire,that they are not damaged and that they are in proper woWng condition;
v. any,vegetation associated with post-coflstriactibil[ BMPs is,suitably established to
w[thstand erosion.
c) The landowner is responskite for BMIP maintenance until the issuing aLlthority is notified
that another party has, logally assurned responsibility for BW rnaifltenance. Prior to
requesting a Certificate of Compliance, or Partial Certificate rat Cornphance, the responsible.
party (defined in General Condition 18(e)) shall execute and submit to the issuing authorlity
an Operation and Maintenance Compliance Statement ("OW Statement) for t`he
Stormwater BMPs identifying the party responsible for implementing the storm water BMP
Operafion and Maintenance Plan C'O&M Plan") and certifying the f0bw ing:.
i.)the O,&M Plan is cornpliete and will be implemented upon receipt of the Gertifibate of
Compliance, and
