Ottenheimer DATE OF FINAL GRADE INSPECTION: 7/12/2022 S.Casey SITE CONDITIONS Contractor reports any changes to design plan Existing septic tank properly abandoned, if present Internal plumbing all to one building sewer Topography not appreciably altered Comments: SEPTIC TANK Building sewer in continuous grade, on compacted firm base Cleanouts per plan Bottom of tank hole has 6” stone base Tank size and loading per plan Monolithic tank construction If monolithic, water tightness of tank has been achieved by visual testing If 2-piece, water tightness of tank has been achieved by water / vacuum test (circle one) Inlet tee installed, centered under access port Outlet tee installed, centered under access port effluent filter installed If effluent filter, riser to finished grade over filter Riser with cover to within 6” of finish grade installed over one access port Manhole and riser minimum 20 inch diameter Pipe penetrations appear watertight using rubber boots Comments: 06.10.2022 S. Casey @ 12pm – 6” of stone observed 06.13.222 installer to dig pump home PUMP CHAMBER (Delete if not present) Bottom of tank hole has 6" stone base Weep hole plugged 1000 gallon Pump Chamber installed H-10 loading Monolithic tank construction Inlet tee installed, centered under access port Pump(s) installed on stable base Alarm float working Pump On/Off floats working Drain hole in pressure line 24” cover at final grade installed over pump access port Water tightness of tank has been achieved by Visual testing Neoprene boots around inlet & outlet Comments: 06.13.2022 @ 12pm >6 inches observed 6-25-22 D. Ottenheimer – No water in tank to demonstrate operation of pump CONTROL PANEL (Delete if not present) Alarm & Pump are on separate circuits Visual & audible alarm works as intended Location of control panel: Basement Comments: DISTRIBUTION-BOX Installed on stable stone base H-20 D-Box Inlet tee (if pumped or >0.08’/foot) Hydraulic cement around inlet & outlets Even distribution observed Schedule 40 PVC Pipe Comments: SOIL ABSORPTION SYSTEM (General) Bottom of SAS excavated down to specified soil layer, as provided on plan Size of SAS excavated as per plan Sand installed, if specified on plan Sand displays few fines when handled 40 Mil HDPE barrier installed, if specified on design plan Laterals installed and ends capped or connected to header (and vented if impervious material above) Retaining wall material, location and height if specified on plan Comments: 06.16.2022 c-layer visible, sand good, installer to address one spot of a-layer – image received 06.20.2022 S. Casey FINAL GRADE Final cover as per plan Loamed Seeded Cover per plan Comments: 07.11.2022 @ 2.20 pm S.Casey could not enter backyard to see tank covers on 7.12.2022 4 covers to grade & inspected port to grade, Pass DOCUMENTS NEEDED Certification of Installation Form submitted by designer and signed and dated by designer and installer As-Built Plan by designer 6-25-22 D.
