No variance was intended to be sought for to site the soil absorption system. Though a small portion of the reserve trench is located within the 100 ft buffer zone, the system was shifted so that the entire reserve trench is located outside of the buffer zone. 2. In providing a scale profile, the existing cross section was inadvertently removed. A scaled cross section has been placed back on the plan. 3. Perc test#3 is rated at 31 minutes for the last 3 inches which equates to 10 minute and 20 seconds per inch, as indicated. The design calculations state the soil absorption system is based on 11 minutes per inch. So that no confusion is assumed and for accuracy purposes, the perc rate is listed as 10 minutes and 20 seconds per inch. 4. On sheet 2 of 2, the septic tank within the profile is previously noted to bring both access covers within 6" of the finish grade. Due to plan legibility on the profile and for contractor clarity purposes, the risers were not drawn. I presumed that the note would be sufficient. Both risers, in conjunction with the note, have been added per the Board of Health comment. 5. On sheet 2 of 2, the plan view and profile previously showed a hatched area of organic soils to be removed within the soil absorption area plus 5 ft around it. Per the Board of Health comment, a note in conjunction with the hatched area has been added to state that organic soils(A and Bw) are to be removed. Unfortunately,the plan cannot be approved until the following items are corrected. The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by this design follows each item where applicable. 1. The name and address of the designer is required to be on the design plan(NA 3.2). 2. Provide a statement identifying whether the property is within or not within the Lake Cochichwick watershed(NA 3.2). 3. Provide the elevation/location statement as described in section 3.2 of the North Andover Board of Health regulations. 4. Indicate if there are any public or private wells within 100' of the proposed soil absorption system. 5. A note or table is required to list all the variance(s)being requested(NA 3.2). It appears a local variance is needed to site a soil absorption system within 100' to a wetland resource area. 6. Indicate that the proposed septic tank shall be a monolithic tank(NA 3.2). 7. Indicate the manufacturer and model numbers of the septic tank and distribution box(NA 3.2). 8. H-20 loading distribution box is required(NA 3.2). 9. Indicate the location and elevation of the proposed foundation drain (NA 3.2). Indicate the location of the proposed water service or potable private well (310 CMR 15.220(4)(m)). 11. A complete scaled profile of the system of no less than 1"=2' vertical and 1"-20' horizontal is required(NA 3.2). 12. Indicate the DEP approved effluent filter brand and model. Also indicate the required annual maintenance(310 CMR 15.227(7)). 13. The soil evaluation forms for TP3-16 through TP6-16 were not submitted(NA 3.2). 14. TP-1 and TP-2 shown within the proposed soil absorption system cannot be used as an acceptable test pits for the purposes of 310 CMR 15.102(2). The test pit was conducted prior to the adoption of the current Title 5 - 310 CMR 15.000 regulation. In addition this office would be concerned with the close proximity of the proposed soil absorption system adjacent to TP4-16 which had bedrock noted at 24"on the southern side of the test pit. It appears the soil absorption system could be situated within the area of TP1-16, P-1, TP2-16,TP3-16 and P-2. A system proposed within this area would meet the requirements of 310 CMR 15.102(2). 15. An inspection port is required within the proposed soil absorption system(310 CMR 240(13)). 16. Sch 40 PVC pipes are required within the soil absorption system(NA 3.2). 17. The design plan should clearly indicate the soil horizons to be removed beneath the soil absorption system. For dwellings with basements,the foundation is assumed to consist of spread footings of reinforced concrete built on undisturbed soil at a depth of about 7 feet. The ratings for dwellings are based on the soil properties that affect the capacity of the soil to support a load without movement and on the properties that affect excavation and construction costs.The properties that affect the load- supporting capacity include depth to a water table,ponding,Flooding,subsidence,linear extensibility(shrink-swell potential),and compressibility.Compressibility is inferred from the Unified classification of the soil.The properties that affect the ease and amount of excavation include depth to a water table,ponding,Flooding,slope,depth to bedrock or a cemented pan,hardness of bedrock or a cemented pan,and the amount and size of rock fragments. The ratings are both verbal and numerical.Rating class terms indicate the extent to which the soils are limited by all of the soil features that affect the specified use."Not limited"indicates that the soil has features that are very favorable for the specified use.Good performance and very low maintenance can be expected."Somewhat limited"indicates that the soil has features that are moderately favorable for the specified use.The limitations can be overcome or minimized by special planning,design,or installation.Fair performance and moderate maintenance can be expected."Very limited"indicates that the soil has one or more features that are unfavorable for the specified use.The limitations generally cannot be overcome without major soil reclamation,special design,or expensive installation procedures.Poor performance and high maintenance can be expected.
