Re: 1230 Salem Street, Map 106A, Lot 41
Dear Board of Health Dept.:
In response to your letter dated July 14, 2016 for the Subsurface Disposal System Plan,
I am providing the following supplemental information.
1. The name and address of the design Engineer have been added to the plan.
2. Though the subject property is outside of the watershed area, a statement
identifying that the property is not located within the lake Cochichwick
watershed has been added to the plan. See note #20.
3. Though specification note#15 states that a boundary property line and
topography, which reflects existing features and ties, was performed by the
Design Engineer, an elevation/location statement has been added to
supplement the plan.
4. There are no known public or private wells within 100 ft of the proposed
subsurface disposal system.
5. With the additional soil testing in April 2017, the soil absorption field was
rotated so that the entire leaching field is outside of the 100 ft wetland buffer
zone. As a result, no local variance will be requested.
6. A note indicating monolithic tank has been added to the plan. See note #22.
7. Detail information on the septic tank and D. Box has been added to the plan.
See note#22.
8. The D. Box is now shown to be H-20 rated. See note #23.
9. A foundation drain with an elevation has been added to the plan. See note #24.
10. A proposed water service has been added to the plan.
11. The profile system has been revised to reflect a 1"=2' vertical and 1"=20 ft
horizontal scaled profile. See sheet 2 of 2.
12. A note indicating a DEP approved effluent filter and schedule maintenance has
been added to the plan. See note #25. With various DEP approved filters
available, no model number or type was selected. Upon the final as-built of the
leaching system in obtaining the Certificate of Compliance, the selection of the
effluent filter shall be confirmed as a DEP approved type. It is the intent to

provide an effluent filter within the PVC outlet tee as 310 CMR 15.227(7) states
that effluent filters may be installed in lieu of outlet tees.
13. Though all 2016 soil test pit and percolation results on DEP Form 11 and Form
12 along with the plan and supporting supplemental information were
submitted for plan review, a full complement of soil testing results including
the additional testing of 2017 are included within this response.
14. It is understood that test pit#1 and#2 dated 1985 are not acceptable as they
predate the current Title 5 and soil evaluation procedures. They are solely
provided for supplemental information only to confirm the depth and presence
of ledge. It is my belief that, with more details and information, the less chance
of a problem to arise. With this supplemental information being provided,
additional soil testing was performed in April 2017 to comply with 310 CMR
15.102(2). Test pit #1-17 was dug in the area of TP#1(1985) to confirm the
absence of ledge and TP #4-16 to show that ledge dives deep on the southerly
side.
15. An inspection port has been added to the plan.
16. All piping within the leaching profile, from the house to the end of the
trenches indicate SCH 40 PVC pipe, as shown on the profile plan.
17. The profile indicates the area of organic soil to be removed from beneath and 5
feet beyond the proposed leaching field.
