Dear Mr. Side.ri:
The proposed onsite wastewater system design plan for the above site dated 7/29/22 with a final
revision elate of 9/22/2.2 has been approved for issuance of a:Disposal System ( onstruction
Permit(I)SC I'). Once issued, a l)SCP will authorize the construction of the onsite wastewater
system per the plan refarcnccd above and as allowed under'Title 5 of the Massachusetts
F'nvironmental Code (31 Cl ("MR 15.000). You will need involvement frorn the designer of the
onsite wastewater system throughout the installation process so you should be were to coordinate
with them.
The design has been approved f'or use in the construction of an upgrade on-site wastewater
system for a 4-bedroorn (rnaximurrr 9-room) home utilizing the proposed septic tank with
effluent filter in the septic tank, pump tank with pump and a standard stone-and-pipe soil
absorption system as indicated on the design plan.
This design plan approval is valid for 2 years from the above dale. During this time, a licensed
septic system installer should obtain a permit and complete this work, and a Certificate of
Compliance be endorsed by the installer, designer, and the Town of North Andover. If these
activities are not completed within this time period then it is possible a new design plan will need.

This approval is subject to the 161lowing conditions. Please be sure to review these with the
designer and installer.
Prior to the issuance of Disposal System Construction permit.:
a. The area of the proposed Soil Absorption System (SAS) is to be staked and
flagged to identify the location(s) and to prevent activities which might damage
the soil absorption system. Stockpiling of rilateriats and equipment within the
SAS area is prohibited
Throughout the project.
a. If site conditions are hound in the -field to be different from those indicated oil the
design plan, installation of the onsite wastewater system shall stop and the
designer and this office notified within 24 hours
b. The brand and model Of effluent. filter specified on the design plan is to be
utilized. The installer is to demonstrate these have been made part of the onsite
wastewater system.
c. If groundwater is present at the bottom portion of the, excavation for the soil
absorption system (SAS) the excavation is to be dewatered prior to the placement
of any fill. No fill is to be placed for the SAS during rain or snow stornis. Prior to
placement of any needed rill in the SAS, the bottom is to be scarified and
relatively dry and the fill material is to be stockpiled at the edge of tile excavation
and filled in gradually
d. Inspections are to be performed by the designer of the onsite wastewater system
as needed to properly oversee and document construction. At a jilininnini these
are the initial excavation, prior to backfilling, and during backfilling as well as
any other component of the construction which the designer wishes to examine
e. Inspections are to be completed by the Town of North Andover at a minimum as
follows:
i. Bed bottom
ii. Tank hottorn
iii. Final installation ofcomponents, confirmation of'proper component
selection, examination ofconstruction methodology, and system operation
iv. Final grade
v. Sand fill will be collected From at least orle location and analyzed far
compliance with specifications in Title 5
f. No components are to be covered until a final component inspection has been
completed
g. No excavation for or work associated with construction of the soil absorption
system is to occur from November 30 to April I
After construction:
h. Within 30 days Of completion of construction the installer and designer are to
confirm if the installation work was completed in compliance with the design plan
referenced in the DSCP and with Title 5, and if so shall certify this oil a

i. It'changes have been made to the appro\cd Plans then preparation ofan as-built
plan is required to be completed by the designer. If no changes have been made
to the approvcd plans then the approved plan shall be modified by the designer to
show thc distances from a known StrLICtUrC, to the system components.
j. No sewage shall be discharged to the onsitc wastewater sysletri until it Certificate
of'Cornpliance has been issued by this off-ice
In addition, you sliould be aware ofthe long-term operatiOn gUidelines ofan onsite wastewater
system (these are nott part ofthe DS(T but arc mentioned below as information for the property
Owner):
I. An onsite wastewater sYstern can function as an integral part of the infrastructure of
the propcily if it is properly operated and n ia.i ntai tied. This is the responsibility ofthe
owner of the property and is generally not specified by regulation. You are
encouraged to contact as wastewater systeni purriper/service provider to lxtrtner with
YOU 10 implement steps needed to assure the IC)DgC\,ity of the onsite wastewater
system being installed at this location
2. '1 fie design Plan specifies the use of an Cf`fIUCDt filter inside the tank. This component
is recluircd to be examined at least annually and cleaned as needed
The issuance ofa DSCP does not COnStRie and/or imply compliance with any Permits, approvals
or licenses which might be needed from other agencies such as the Conservation ('ornmission,
/oning Board, planning Board, Building Inspector, plumbing Inspector and/or Electrical
Inspector.
