New BM established by engineer according to the installer. – D. Ottenheimer SEPTIC TANK Building sewer in continuous grade, on compacted firm base Cleanouts per plan Bottom of tank hole has 6” stone base Tank size and loading per plan Monolithic tank construction If monolithic, water tightness of tank has been achieved by visual testing If 2-piece, water tightness of tank has been achieved by water / vacuum test (circle one) Inlet tee installed, centered under access port Outlet tee installed, centered under access port Gas baffle installed If effluent filter, riser to finished grade over filter Riser with cover to within 6” of finish grade installed over one access port Manhole and riser minimum ___ inch diameter Pipe penetrations appear watertight using rubber boots Comments: 7.18.2022 new tank location due to ledge. Engineer notified BOH of change per inspection. 6” stone noted in tank hole. 7/26/22 – Two-compartment tank, proper tees installed on center wall – D. Ottenheimer DISTRIBUTION-BOX Installed on stable stone base H-20 D-Box Inlet tee (if pumped or >0.08’/foot) Hydraulic cement around inlet & outlets Even distribution observed Schedule 40 PVC Pipe Comments: SOIL ABSORPTION SYSTEM (General) Bottom of SAS excavated down to specified soil layer, as provided on plan Size of SAS excavated as per plan Sand installed, if specified on plan Sand displays few fines when handled 40 Mil HDPE barrier installed, if specified on design plan Laterals installed and ends capped or connected to header (and vented if impervious material above) Retaining wall material, location and height if specified on plan Comments: 07.01.2022 system 47x28’ depth good c-layer 07.06.2022 sand good SOIL ABSORPTION SYSTEM (Enviro-septic) Brand and Model of Chamber: correct Number of chambers per row and number of rows per design plan: correct Elevations of laterals and chambers installed as on approved plan Comments: 7/26/22 – Enviro-septic soil absorption system appears to be built per plan. Plan calls for drain holes in down slope corner of vent pipe. Requested installer or designer to confirm this is allowed by Enviro-septic. Also, plan says installer is to determine if roof vent is adequate for use as a high vent or not, this needs to be determined. – D. Ottenheimer FINAL GRADE 8.23.2022 S.
