Allen & Major Associates, Inc., herewith submits an application and supporting documentation
for Site flan Review for the property located at 1812 Turnpike Street and owned by Jean Paul
Proulx.
An application for Site Plan Review for this project was previously submitted on January 18,
2000, and then subsequently withdrawn at the request of the North Andover Planning Board on
January 16, 2001. Several items from this previous submittal remain unchanged and are
included by reference herein. These items are as follows:
• Traffic Report prepared by Highway & Traffic Signal Design, Inc.
• Application for Permit to Access State Highway
• Community and Fiscal Impact Statements prepared by PD Associates, LLC
The following materials have been updated and/or modified,to reflect the changes requested by
the North Andover Conservation Commission and the consultant to both the Conservation
Commission and the North Andover Planning Board, Coler&"Colantonio, Inc.:
• Site Plan Application
• Site Plan prepared by Allen& Major Associates, Inc.,'dated August 3, 2001.
• Drainage Report prepared by Allen & Major Associates, Inc., dated September 24,
2001.
• Correspondence from John C. Chessia, P.E., of Coler & Colantonio, indicating that'
his review of the Site Plan and Drainage Report (with revisions) is complete and that
all of his outstanding concerns have been satisfactorily addressed.
The submittal package was assembled in accordance with the Application Check List for Site
Plan Review as well as Section 8.3.5 of the North Andover Zoning Bylaws.
