The Division of Public Works has reviewed the above referenced plan by Allen&Major Associates dated
--_October 12,2001.- We have the following.-comments: ---- - ----
1. All sewer lines shall be installed in-'/"9One, from 6"below invert to the top of-the pipe. The trench
detail for sewer/drain pipe installation on sheet D-1 must be changed.
2. The sewer line under Turnpike Street to SMH#1 shall be 8"diameter and will be a public sewer after
all required testing has been performed, The sewer lines from SMH#1 to the building will be
privately owned and maintained.
3. All sewer tines must pass leakage and deflection tests,
4. The proposed sewer lines are all 6"PVC at.S%slope. Minimum slope for 6"PVC is 1%. The
designer must increase the slope to 1%,or increase all pipe diameters to 8".
5. The sewer line from SMH#1 to SMH#2 cannot be installed under the proposed retaining wall. It
appears the sewer lhie can be moved into the driveway entrance while maintaining-i0' separation: rom
water lines.
6. Sewer mitigation fees apply to this facilfty.
7. Change item#14 of the Water Notes on sheet ABB to:The 6"fireline must pass a pressure test of 200 -
PSI for 1 hour.
8. A Massachusetts approved testable double check valve assembly backflow preventor must be installed
on the firelfne.

I have reviewed the plans for the proposed development at the referred location and have the following
comments.
Pursuant to the Zoning By-law the front setback along Route 114 Shall be a minimum of 100 feet. The
setback on Drawing C-2 is 93 feet as the building footprint indicates a building overhang of 8 feet over the
foundation. A petition for a variance must be submitted to the Zoning Board of Appeals(ZBA).
The plan shows varying driveway widths from 18 feet to 24 feet.The Zoning By-law requires unobstructed
driveways not less than 25 feet wide. A ZBA variance is required.
Foot Note—1,Paragraph -2 reads in part"Adjacent to residential districts,an additional 15 foot side or
rear set back shall be required The first 15 feet of the total setback abutting the residential district shall
remain open and green,be suitably landscaped,unbuilt upon,unpaved and not parked upon'. The plan
does not indicate compliance with this buffer requirement at the rear lot line.
No elevation or floor plans have been submitted to the Zoning Enforcement Officer. It is impossible,at
this time,to determine if the building height requirement has been met. Without floor plans of the building
indicating the type and square footage of retail and office space/use,it is not possible to determine the
number of required parking spaces.
The Pylon Sign as shown on Drawing C-2,is to be referred for sign by-law compliance to the Building
Commissioner/Zoning Enforcement Officer for permitting.
It is important that the petitioner not be allowed to pile snow in what appears to be an inadequate parking
area.

I have reviewed the plans for the proposed Office/Retail building to be located at 1812 Turnpike St., stamped by
Benjamin Smith dated 12104/01,
With reference to the NOTES for these plans:
• Earth Removal items 1,2,3 discuss blasting operations in accordance with State Law. The fire department will
require that a detail be present during blasting operations in accordance with the State Fire Prevention Code 527
Section 13.
• The building shall required a complete fire suppression and protection system. These systems shall be connected to
the municipal fire alarm system through the installation of master fire alarm box.
• In Water Notes item 20-an additional fire hydrant will be required adjacent to the building but the location of that
hydrant will be dependent upon the location of the installation of the fire protection water main which is not as yet
determined.
• The fire department will witness all tests of water mains to be used for fire protection.
• The access to this building is in conformance with 527 CMR 25 and the access design as indicated on the plan
provides adequate access to the building.
These are our comments at this preliminary permitting stage. The fire department does not object to the plans as
shown in considering fire protection and safety.
We Will work with the applicant for more specific information regarding fire protection systems as this project
progresses.

My engineer has already forwarded plans to your office and DPW for review. I have
instructed Paul Proulx to post a $10,000,00 dollar bond for the purpose of ensuring that a
final as-built plan is submitted.

EASEMENT TERMS
Within the Drainage Easement Area, Lot A shall have the permanent right and easement
to construct, install, maintain, operate and repair drainage basins and swales, drainage
pipes and structures for its benefit, together with any and all appurtenances thereto as the
same are needed to effectuate the operation of said drainage swales, pipes and structures.
Also conveyed herewith is the right and Easement within the Easement Area to remove
trees, bushes undergrowth, fencing and other obstructions interfering with the location,
construction, maintenance and replacement of said drainage basins and swales, pipes and
structures. The"Grantor", for itself and its successors and assigns, reserves the right to
grant additional easements within the Easement Area as long as such additional

I have reviewed Allen&Major Associates, Inc. correspondence of November 13`h on the referred
project(copy enclosed)and will specifically answer by their number the response to my letter of
November 6, 2001.
1. A shorter cantilevered overhang is not allowed as it is built as part of the
structure and in violation of the setback. See Exhibit#1 for an example of a
cantilever.
An eave is part of the ventilation system of the roof only and cannot be exempt from
setback regulations when built on to the building. See Exhibit#2 and#3 for an eave.
A canvas or wood frame awning will have to be fastened to the structure and
in violation of the setback. See Exhibit 44 for an example of an awning
frame.
A petition for a variance must be submitted to the Zoning Board of Appeals(ZBA)for any of the
above.
2. The Zoning By-law specifically states that driveways are to be 25 feet wide. The by-
law does not indicate that the width can be lesser because it is designated as ONE
WAY. The ZBA is the only Board that can vary this dimension and it must be
petitioned for this variance.
4. The Building Commissioner/Zoning Officer,to this,date has not seen
the floor plan designating the type of retail or business to occupy the structure.
Pursuant to Section 8 the parking will be estimated at the maximum required for this
project. Site Plan C-2 indicates 20,000 S.F. for both—therefore 300 parking spaces
must be supplied.
