Dear Mr. Abele:
'I"he proposed onsite wastewater systein design plan for the above site elated 7/8/22 with a final
revision date of 1.1/1/22 has been.approved for issuance of a Disposal System Construction
Permit (DSCI'). Once issued, a DSC'P will authorize the construction of"the onsite wastewater
system per the plan referenced above and as allowed under'l"'itle .5 of the:Massachusetts
Environmental Code (31.0 CMl2. 15.000). You will. need involvement from the designer of the
onsite wastewater systern throughout the installation process so YOU should be sure to coordinate
with them.
The design has been approved .for use in the construction of an upgrade on-site wastewater
system for a 3-bedroorn home utilizing the existing septic tank and a standard stone-and-pipe soil
absorption system as indicated on the design plan.
This design plan approval is valid for 2 years fro:rn the above date. During this time, a licensed
septic system installer should obtain a permit and complete this work., and a Certificate of`
Compliance be endorsed by the installer, designer, and the Town of North Andover. If these
activities are not completed within this time period then it is possible a new design plan will need
to be completed and submitted for review, In the event an imminent health problem such as
sewage backup into the dwelling is occurring the North Andover Board of Health may reduce the
tame period for which this plan is valid.
Tlxas approval is subject to the following conditions. Please be sure to review these with the
designer and installer.

In addition, you should be aware of"the long-term operation gUidChlleS Ofau onsite wastewater
system (these are not part of the DSCJ1 but are tnentioned below as information for the property
owner):
I. An onsite wastewater system can function as ain integral part of the infrastructure of
the property if it is properly operatcd and maintained. This is the, responsibility of the
owner ol'the property and is generally not specified by regulation. You are
encouraged to contact a wastewater system PLUTIper/service provider to partner with
you to implenient, steps needed to cISSUre the lonV,evity of the onsite wastewater
system being installed at this location
The issuance of"a DSCP does not constrUe and/or imply compliance with any peri-nits, approvals
or licenses which might be needed frorn other agencies such as the Conservation Commission,
Zoning Board, planning Board, Building Insf.,)ector, Plumbing Inspector and/or Ellectrical
Inspector.
