The inspection revealed violations of the State Sanitary Code,Chapter II as listed on the attached Violation Form. You are hereby ORDERED to correct the violations within the time allotted on the enclosed form. Failure to comply within the specified time period will result in further enforcement actions including but not limited to a fine of up to$500 per day'in accordance with 105 CMR 410.910 of the State Sanitary Code or filing in court, as well as issuance of a condemnation order and order to vacate the unit. Fines will continue to accrue until the subject property is brought into compliance with this Order to Correct. Each day or portion thereof during which the violations continue shall constitute a separate offense. You have the right to request a hearing before the Board of Health if you feel this Order to Correct should be modified or withdrawn. A request for said hearing must be made in writing and received by the Health Department within seven(7) days from receipt of this Order. At said hearing you will be given an opportunity to be heard and to present witnesses and documentary evidence as to why this Order should be modified or withdrawn. Investigate and repair source of water . Days damage, and remediate water damage. 410.500 Metal thresholds on floor throughout the home had come loose 30 N and were in disrepair in multiple areas. This includes in the Days doorways between the front entry way, living room, and the first-floor bedroom..Repair or replace these thresholds. 410.505 Holes were present in living room wall. Repair holes in wall. 30 N Occupant Days Responsibility 410.351 Outlet covers in living room and dining room were broken or 30 N missing. Repair or replace outlet covers. Days 410.351 Light fixtures in numerous rooms were not working properly, 5 N and either portions of the light fixture or the fixture as a whole Days did not turn on when activated. This includes in the front entry way, living room,first floor bedroom, dining room, and second floor bedroom. Repair or replace light fixtures to work as intended. 410.500 Door for the first-floor bedroom was not shutting properly and 14 N had very loose hinges. Repair or replace door to shut and latch Days properly. 410.351 Light fixture in dining room was missing cover. Repair or 14 N replace cover. Repair or replace so floor is free of defects, Days and so floor is smooth,noncorrosive,nonabsorbent and wate roof. 410.351 Kitchen sink hot water does not shut off when turned on. 7 N Occupant has to use shutoff valve under sink to turn hot water Days off. Re air or replace sink to work as intended. 410.500 Cabinet under sink has large hole in base of it. Repair or 30 N replace to be free of hole. Days 410.500 Bathroom floor is in disrepair. Tile placed over wooden floor 5 N 410.504 has come off completely in many areas, and a hole is present Days going under shower. Repair or replace bathroom floor to be free of defect, and so floor is smooth,noncorrosive, nonabsorbent and waterproof. 410.351 Bathroom light fixture is missing light cover. Provide cover for 14 N light fixture in bathroom. Days 410.500 Bathroom ceiling has peeling paint across it. Repair or replace 30 N to be free of defects and peeling paint. Days 410.500 Stairs leading to second floor are in poor condition—second 5 N stair from the top is broken and others feel wobbly when Days ste ed u on,nails near bottom are protruding through step. 410.505 Second bedroom upstairs had hole/defect in wall. Repair or 30 N Occupant replace wall to be free of defect. All doors leading outside of the dwelling unit need to be able to be secure against unlawful entry. Provide lock on this door The occupant raised concerns over lead paint due to a child under 6 residing in the dwelling unit. Our research shows the building was constructed after 1978, and was built in 1984, and therefore a lead paint determination is not required.
