HomeMy WebLinkAboutUpdated Septic Approval Letter - Correspondence - 24 FARNUM STREET 10/12/2022 16 North Andover Health Department Community and 1(onomic Development Division October 12, 2022 Warren Sideri 24 Farnum Street North Andover, MA 01845 Re: Subsurface Sewage Disposal System Plan for 24 Farnum Street (Map 107A, Lot 24) Dear Mr. Sideri: The proposed onsite wastewater system design plan for the above site dated 7/29/22 with a final revision date of 10/06/22 has been approved for issuance of a Disposal System Construction Permit (DSCP). Once issued, a DSCP will authorize the construction of the onsite wastewater system per the plan referenced above and as allowed under Title 5 of the Massachusetts Environmental Code (310 CMR 15.000). You will need involvement from the designer of the onsite wastewater system throughout the installation process so you should be sure to coordinate with them. The design has been approved for use in the construction of an upgrade on-site wastewater system for a 4-bedroom (maxim urn 9-room) home utilizing the proposed septic tank with effluent filter in the septic tank, pump tank with pump and a standard stone-and-pipe soil absorption system as indicated on the design plan. This design plan approval is valid for 2 years from the above date. During this time, a licensed septic system installer should obtain a permit and complete this work, and a Certificate of Compliance be endorsed by the installer, designer, and the Town of North Andover. If these activities are not completed within this time period then it is possible a new design plan will need to be completed and submitted for review. In the event an imminent health problem such as sewage backup into the dwelling is occurring the North Andover Board of Health may reduce the time period for which this plan is valid. Page I of 3 North Andover Health Department, Town Hall, 120 Main Street North Andover, MA 01845 Phone: 978.688.9540 Fax: 978.688. 9542 This approval is subject to the following conditions. Please be sure to review these with the designer and installer. Prior to the issuance of a Disposal System Construction Permit: a. The area of the proposed Soil Absorption System (SAS) is to be staked and flagged to identify the location(s)and to prevent activities which might damage the soil absorption system. Stockpiling of materials and equipment within the SAS area is prohibited Throughout the project: a. If site conditions are found in the field to be different from those indicated on the design plan, installation of the onsite wastewater system shall stop and the designer and this office notified within 24 hours b. The brand and model of effluent filter specified on the design plan is to be utilized. The installer is to demonstrate these have been made part of the onsite wastewater system c. If groundwater is present at the bottom portion of the excavation for the soil absorption system (SAS)the excavation is to be dewatered prior to the placement of any fill. No fill is to be placed for the SAS during rain or snow storms. Prior to placement of any needed fill in the SAS, the bottom is to be scarified and relatively dry and the fill material is to be stockpiled at the edge of the excavation and filled in gradually d. Inspections are to be performed by the designer of the onsite wastewater system as needed to properly oversee and document construction. At a minimum these are the initial excavation, prior to backfilling, and during backfilling as well as any other component of the construction which the designer wishes to examine e. Inspections are to be completed by the Town of North Andover at a minimum as follows: i. Bed bottom ii. Tank bottom iii. Final installation of components, confirmation of proper component selection, examination of construction methodology, and system operation iv. Final grade v. Sand fill will be collected from at least one location and analyzed for compliance with specifications in Title 5 f. No components are to be covered until a final component inspection has been completed g. No excavation for or work associated with construction of the soil absorption system is to occur from November 30 to April 1 After construction: li. Within 30 days of completion of construction the installer and designer are to confirm if the installation work was completed in compliance with the design plan referenced in the DSCP and with Title 5, and if so shall certify this on a Certificate of Compliance form Page 2 of 3 North Andover Health Department,Town Hall, 120 Main Street, North Andover, MA 01845 Phone: 978.688.9540 Fax: 978.688. 9542 i. If changes have been made to the approved plans then preparation of an as-built plan is required to be completed by the designer. If no changes have been made to the approved plans then the approved plan shall be modified by the designer to show the distances from a known structure to the system components. J. No sewage shall be discharged to the onsite wastewater system until a Certificate of Compliance has been issued by this office In addition, you should be aware of the long-term operation guidelines of an onsite wastewater system (these are not part of the DSCP but are mentioned below as information for the property owner): 1. An onsite wastewater system can function as an integral part of the infrastructure of the property if it is properly operated and maintained. This is the responsibility of the owner of the property and is generally not specified by regulation. You are encouraged to contact a wastewater system pumper/service provider to partner with you to implement steps needed to assure the longevity of the onsite wastewater system being installed at this location 2. The design plan specifies the use of an effluent filter inside the tank. This component is required to be examined at least annually and cleaned as needed The issuance of a DSCP does not construe and/or imply compliance with any permits, approvals or licenses which might be needed from other agencies such as the Conservation Commission, Zoning Board, Planning Board,Building Inspector, Plumbing Inspector and/or Electrical Inspector. Please feel free to contact the office with any questions you may have. We look forward to working with you to obtain a wastewater treatment and dispersal system which will be in compliance with all regulations and assure protection of public health and the environment of North Andover. This reflects that change, and I would have printed it but the new printer isn't working this second! If you can print out this copy for the file that'd be great. I added it to the septic plan submission and mailed it to the homeowner. That is a Cons Comm issue, not a Health Department issue, so I see no reason we need to review this revised plan (unless you want me to) https://mail.google.com/mail/u/0/?ik=aOc6f4e4cf&view=pt&search=all&permthid=thread-a°/a3Ar913568027386143907&simpl=msg-a%3Ar3868650114... 1/3 10/17/22,8:42 AM Town of North Andover Mail-24 Farnum Street-revised submittal Daniel Ottenheimer, PE President MALL RIVER CONSULTING t.:a cati a 5olatimis for I:utd lies elopmrtt 6 Sargent Street Gloucester, MA 01930-2719 Phone: 978-282-0014 ext.802 www.millriverconsulting.com From: Toni Wolfenden <twolfenden@northandoverma.gov> Sent: Friday, October 7, 2022 11:31 AM To: Daniel Ottenheimer<dano@millriverconsulting.com>; Brian LaGrasse <blagrasse@northandoverma.gov>; Stephen Casey<scasey@northandoverma.gov> Subject: 24 Farnum Street- revised submittal Hi Dan, Please find attached the revised plan for 24 Farnum Street.
