Attached to this cover letter is all outline oftoday's inspection findings as well as some images from the inspection. Despite numerous requests to nraintaain as cleaning crew on site at as nainirraaina of'3 times per week and pest control city site. at 3 times per week, after speaking with the rraanaager and staf`f`on site today it was noted a cleaning crew had not been ore site since Thursday, March 1 O'r' 2023, which rnearis from March 1"7'n' to March 22""t there, was no cleaning, crew present earl site. In addition, it was reported that the hest control company was presenting the rrraraager with issues regarding mass trappings and suhsealarent follow ups to address the mice issue. Because oCthe, worsening conditions and continued rodent int'cstartion ongoing at the property, this issue is creating coraditic:tns constituting as laaaBalic health nuisance. In accordance with Massachusetts General Laaw (:'haapter :N.11, Section 122,year are hereby ordered to submit as signed contract with as professional cleaning, company to be onsite 5 times per week and with as licensed hest control company to be onsite 3 times per weep by March 30111,2023. These companies must work jointly with PeWo to abate the hest; infestation at the above location by no later than April cat", 2023. Farilure, to abate the issue by the specified date will result in braes of'up to S1,000 per(lay, closure of your establishment and other potential enforcement actions. Regular Mail Inspection Findings— March 22, 2023 During the inspection.of the ongoing rodent infestation at North Andover IletCo location on the above date,the -following issues were noted; • Wliile in the dog food aisles, a mouse was spotted running across the floor towards the center or the store. In addition, numerous droppings were noted throughout these aisles. 'The aisles or shelving and wall shelving containing the brands Royal Canin, Purina ProPlan, Hills, Taste of Wild, Whole,hearted.,Wellness Core RaWRev, and Blue Buffalo all had substantial arnowits of droppings on the shelving itself and the floors around the shelving. • In the Reptile Aisle very few droppings were seen on the floor and around shelving, however there were mobile shelving units and boxes stored in this small area. ']I"here were numerous droppings noted in between the wall shelving where the reptile area.meets the dog:food wall shelving. • The Aquatics aisles had numerous droppings in the third aisle from the Reptile area. `These were present around buckets of"Instant Ocean"product at the end of the aisles. In addition,there were droppings noted in front of the wall aquariums on the floor. • The (,enter Aisles had light amounts of droppings noted on the floors arid she]ving nearest the Aquatics area. I also noted rouglily 3-4 droppings where the Stella& (..hewy products were, arid about 5-1.0 near the Stella& Chewy Wild Red dog food cans. • In the Cat Aisle, there were four cleaners working on the first two aisles when I arrived. That being said, I inspected the rest of the aisles containing cat food and prod LiCt and noted there were numerous dropping present on the shelving and floors. The aisles I noted this in were the brands titled Royal Canin, Hill, Tild Cat Born,and Wellness, Core, which also had evidence of shredding of cardboard and bags.. • The Bird food area had a bag of bird seed torn open and seed was spread among the shelf. • The Rear Loading Area had 2 dead rnice in snali traps on the floor. There were numerous droppings noted all around the base of the walls. This area has comnionty been the problern area along with the Dog Food and Cat Food areas. • The Water Heater Closet had droppings present on cardboard boxes and around the water heater. • The Break Room had numerous droppings present behind the door into the room. 1"here were a, flew droppings near the mini fridge as well. All these citations contribute to the creation of a public health nuisance at this establishment.
