The Applicant has addressed VHB's comments and no further engineering review is required at this
time.
The plan was reviewed for conformance to the 1972 Town of North Andover Zoning Bylaw(last amended
July 10,2006). The Applicant has submitted the following information for VHB's 3rd review:

The following comments note non-conformance with specific sections of the Zoning Bylaw or
questions/comments on the proposed design and VHB's recommendations/suggestions. Original comments
are shown in regular font,2r`d review comments are shown immediately after in italic font and where
required,follow up comments shown in bold font.
Town of North Andover Zonin B law
I. (8.1.14)This section requires that handicap parking space be 12'x 20'.Plan(sheet 5)indicates the
.handicap parking space dimension to be 9'x 18'.The Applicant should revise.handicap parking space
dimensions.
Not addressed. The Applicant has not revised the space dimensions as required by this section.
Addressed. In an email dated July 8,2008 VHB confirmed the current handicap space
dimensions conform to current ADA regulations,and recommended the Applicant request a
waiver to the current bylaw. VHB assumes that the Applicant will follow through with the Town
regarding this issue.
2. (8.3.5.e.viix and viii)See Drainage Comments
No comment required.
3. (8.3.5.e.xxi)Sewer line locations and profiles are required according to the section, Only sewer line
locations are shown on sheet 6 of 13.Applicant should provide sewer line profile.
Addressed.

4. (8.3.5.e.xix) A traffic study was not included in the submission. Has the Applicant requested a
waiver?
Addressed. The Applicant has stated they will request a waiver from this section. VHB will defer to
the Planning Board in this matter,
5. (8.3,5.e,xxii) A fiscal impact study was not included in the submission, Has the Applicant requested a
waiver?
Addressed The Applicant has stated they will request a waiver from this section. VHB will defer to
the'Planning Board in this matter.
6. (8.3.6.iv) Per this section,the location and number of curb cuts shall be mininuzed to reduce turning
movements and hazardous exits and entrances.The second(northerly)opening appears to be beyond
the proposed lease area line.Applicant should verify if the opening beyond the lease line is acceptable.
Has the Applicant considered consolidating the driveway openings?
Addressed.
General Comments
7. The Applicant should provide a pedestrian ramp for the 5'concrete sidewalk at the two handicapped
parking spaces.
Partially addressed. The Applicant should note that the ramp opening dimensions shown on the ramp
detail on sheet 9 are not consistent. The Applicant should revise as necessary.

8. The"FieldTur#"detail shown on.Sheet 10 of 13 shows several material layers but does not provide the
depth of each layer.The Applicant should provide depths for each layer,
Not addressed The Applicant has removed the Field Turf detail f'om the revised plans. Additionally,
the proposed surface treatment of the fenced in area behind the proposed building is not clear on the
landscape plan(sheet 7). The Applicant should revise the plans as necessary and address the original
comment if thefield turf is.to be the proposed surface treatment.
Addressed. The Applicant has clarified the surface treatment of the fenced-in area behind the
proposed building. Upon further review,the depth of the material layers making up the Field
Turf should be in accordance with the manufacturer's specifications,
9. The."Concrete Curb On-Site"detail shown on sheet 10 of 13 (note 1)calls for 2500 PSI concrete,yet
note I directly below calls for 2000 PSI concrete,The Applicant should coordinate the notes.

10. The location and label for the outlet control structure is not shown on the plans.
Addressed
11. There may be a utility conflict where the underground electric and telephone conduits cross the
underground infiltration system shown on sheet 6 of 13.
Addressed.
12. Roof Drain(RD-2)goes to an underground infiltration system before overflow goes to DMH-1 while
Roof Drain(RD-1)goes directly to DMH-1.Why doesn't RD-1 go into an underground infiltration
system prior to discharging into DMH-1?
Addressed The Applicant may wish to clam,the roof drain inverts as they enter the proposed 24-inch
perforated pipe to avoid confusion during construction.

13. The Applicant should verify that proposed 12"HDPE drain pipe material from DMH-2 to DMH-3
(within public right of way)is acceptable to the Town.The Town may require reinforced concrete pipe
beneath the roadway.

Drainage Comments
15, (Section 8.3.5e)vii)This section states that all storm water drainage control facilities utilized by the
site shall be shown on the site plan and calcuiations.shail show a mitigation of run-off to zero of the 2,
10,and 100 year storm event. The Applicant has submitted stormwater drainage calculations which
support the design of the control facilities shown on the plan.

16. (Section 8.3.5e)xviii)The Applicant has submitted a detailed hydrology study for the site and VHB
comments are as follows:
a. The existing conditions area is separated in two sub-areas;however,Sub-area 2 is not

delineated on the pre and post conditions plans.Addressed
b. Dl 2 is not shown on the plans.Addressed
c. : The Te should start at a defined point on the watershed area in the pre-development plan.

d, The Post-Development narrative states that the analysis takes into account 8 sub-catchment
areas,however,only seven are shown.Addressed
17. VHB suggests that catch basin to catch basin layout be reconsidered. Currently,CB 1 is connected to
CB2 which appears to be a proposed Stormceptor,STC 450i structure. VHB suggests additional drain
manholes be considered to avoid catch basin to catch basin layout.
Not addressed. The plans still show a direct connection from CB2 through the STC 450i structure.It
should be demonstrated that the STC unit can handle the flow from CB2 and FES 3 as well as the
surface runoff collected in its grate.If runoff f•onr any future development will flow through this
system than that should be considered as well.
Addressed, Please note that CB3 is also an Wine catch basin and should be set offline to avoid
potential clogging.
18. The FES-3 should have a trash grate at its end. This would prevent clogging in the new drainage
system. A trash gate detail should be added in the plans.
Addressed.
19. The Applicant should verify that the Town allows an overflow connection into its drainage system as
proposed on the plans.
Not addressed.Additional grading detail at the detention pond overflow should be provided to ensure
that runoff is captured in CB-3.
Addressed. Additional grading has been provided. VHB assumes that the Town has approved
an overflow connection to their drainage system,as stated in our original comment.
20. VHB recommends adding drain manhole detail in the plan set,

21. VHB recommends adding a hood in the catch basin detail.
Addressed, the catch basin detail should be referenced to include the hood.
22, A 24-inch outflow pipe is proposed between CES-2 and DMH-2,but a 12-inch pipe is proposed
between DMH2 and DMH3. The Applicant should demonstrate that the 127inch pipe will be able to
dandle the flows coming from the 24-inch pipe.
Addressed. The included Hydro CAD analysis shows the 12"pipe between DMH2 and DMH3 has
adequate capacity.
The Applicant should provide backup calculations to demonstrate that the existing 12"RCP pipe can
handle added flows into the municipal drainage system at proposed DMH 3.

23. The"Typical Subsurface Infiltration System Layout"detail does not seem to pertain to the proposed
infiltration system shown on the plans.

24. VHB recommends adding a detail of the pipe between the underground infiltration system and DMH-1
to the plans.

26. There are three Flared End Section details shown on the plans. However,there are two flared end
section proposed on the site,FES-1,identified in the"Outlet Protection Flat"detail and FES-3. 1t is
not clear which detail pertains to FES-3 on the detail sheets. The Applicant should identify both Flared
End Sections on the detail sheets.
Not.addressed.FES-1 is included as it is the only FES outlet.Does FES-3 require any protection as an
inlet?The use of the FES symbol and label CES-2 on the plan for what appears to be the outlet control
structure is confusing, The OCS should be clearly labeled on the plan and profile.

27. VHB recommends that a fence be provided around the detention basin for safety reasons.
Partially addressed. A 4'-high chain fink fence has been added to the site plan.A detail should be
provided,
Addressed. According to the Applicant,the Planning Board has requested the fence to be
removed,
28. There is some inconsistency between the Grading Drainage and Utility plan,the details,and the
HydroCAD analysis regarding the outlet invert from the subsurface infiltration system.

29. The CB detail and STC detail need to specify the inlet frame and grate.

Massachusetts DEP„S„tormwater Management Standards
The Applicant has provided the Massachusetts Stormwater Management Form for the original design
showing whether the proposed drainage system is compliant with all 9 Stormwater Management Standards
of the Massachusetts DEP Stormwater Policy Handbook of.1997. The standards are as follows:
a) No new stormwater outfalls via),discharge untreated stormwater directly to or cause
erosion in wetlands or waters of the commonwealth
Not applicable to this project.
b) Stormwater management systems must be designed so that post-development peak
discharge rates do not exceed pre-development peak discharge rates.
Standard met.
c) Loss of annual recharge to groundwater should be minimized through the use of
infiltration measures to the maximum extent applicable. The annual recharge from
post-development site should approximate the annual recharge from the pre-
development or existing site conditions, based on soil types.

d) For new development,stormwater management systems must be designed to remove
80%o the average annual load.f g (post-development conditions)of Total Suspended
Solids(7'SS).

e) Stormwater discharges fr•oin areas with higher potential pollutant loads require the
use of specific stormwater-management BMP's. The use of infiltration practices
without pretreatment is prohibited.
