PIC to provide handwashing signage at handwashing sink. 6-301 .14 REGULATION: Item 10 Adequate handwashing sinks properly supplied and accessible -- Handwashing Signage OBSERVATION: Observed quaternary sanitizer in three bay sink below approved threshold levels per manufacturer's instructions. PIC to use testing strips to ensure sanitizer is maintained at proper concentration levels. 4-501 .114-P REGULATION: Item 16 Food contact surface; cleaned and sanitized -- Warewashing Chemical Sanitization-Temperature, pH Concentration OBSERVATION: Observed chopped tomatoes and shredded lettuce both in cold holding table at 56'F. Observed ice below unit to be low which may be attributing to elevated temperatures. PIC placed TCS foods in walk-in refrigerator during time of inspection. PIC to ensure working containers of chemicals are labeled with their common name. 7-102 .11-Pf REGULATION: Item 28 Toxic substances properly identified, stored and used -- Common Name-Working Containers OBSERVATION: No food thermometer available at time of inspection. PIC states intention to provide food thermometer. 4-302 .12 REGULATION: Item 36 Thermometer provided and accurate -- Food Temperature Measuring Devices OBSERVATION: Observed droppings consistent with mice on multiple non-food contact surfaces including but not limited to bottom shelf below prep table across from fryers, elevated shelves of chemical/dry storage rack, below microwave on elevated shelf, below receipt printer near entrance to kitchen, and on dry storage pallets to left of stairs in basement. Employees starting cleaning and sanitizing affected surfaces mentioned above during time of inspection. 6-501 .111-Pf REGULATION: Item 38 Insects, rodents, and animals not present -- Pests, free of Correct On Site OBSERVATION: Observed droppings consistent with mice in plastic container holding bags of breader & batter mix on bottom shelf of prep table across from fryers. Observed additional droppings in box of instant rice that PIC states is their own personal food. Both food items were voluntarily discarded during time of inspection. 6-501 .111-Pf REGULATION: Item 38 Insects, rodents, and animals not present -- Pests, free of J&M Subs Seafood & Pizza Date: 05/12/2023 Page 4 of 4 DESCRIPTION OF VIOLATION Fail Code Discussion No pest control reports available on site at time of inspection. PIC states intention to contact pest control company for service and for reports. PIC to contact Brian LaGrasse at health office prior to opening establishment on Monday (5/15) to give status update on pest activity and scheduled service. OBSERVATION: Observed ample accumulation of grease and dust behind fryers and on floor below dry storage containers . PIC to ensure area is cleaned at a frequency necessary to decrease such accumulations. 4-602 .13 REGULATION: Item 49 Non - food contact surfaces clean -- Nonfood Contact Surfaces OBSERVATION: Observed ample accumulation of cardboard and bags of recyclable bottles in basement. PIC to remove such articles to reduce attraction of rodents and other pests. 5-502 .11 REGULATION: Item 54 Garbage and refuse properly disposed; facilities maintained -- Frequency-Removal OBSERVATION: Observed old/soiled fryer in basement area that is no longer used in establishment. PIC to remove fryer along with any other articles in basement area that are no longer used in operations of establishment. 6-501 .114 REGULATION: Item 55 Physical facilities installed, maintained, and clean -- Maintaining Premises, Unnecessary Items and Litter OBSERVATION: Due to number and nature of violations cited above, it appears PIC is not performing their duties. 2-102 .11-Pf REGULATION: Item 01 PIC present, demonstrates knowledge, and perform duties -- PIC Demonstration Discussion Observed hood sticker stating next service due April 2023. PIC states hood cleaning company is scheduled to service hood soon. PIC to have date scheduled by re-inspection date. In Compliance Observed CFPM and allergen certificates posted in public view. Observed seafood salad and ham both in top section of in-line refrigerator at 40°F and 41°F respectively. Observed fresh mozzarella in cabinet section of unit at 41°F. Observed cooked peppers and pickles both in "True" branded single-door refrigerator at 40'F and 36'F respectively. Observed packaged raw beef in walk-in refrigerator at 41°F. Observed sausages in table-top hot holding unit at 170°F. Observed shredded chicken in metal pan on flat-top grill at 178°F. Observed foods in freezer units to be frozen solid. Observed clean knives in knife holder. Observed toilet room in compliance. Closing Correct Priority Item and Priority Foundation Item violations immediately; Core Item violations within 10 days. Correct all violations in entirety and maintain. Train and supervise staff. Failure to correct all violations and maintain corrections may result in administrative action and or fines.
