Verified 05/08/23 OBSERVATION: Observed container of raw chicken being stored above cooked
foods in cabinet section of in-line refrigerator across from tandoori
oven. Discussed food storage hierarchy with PIC.
**RE-INSPECTION** OBSERVED CONTAINERS OF RAW CHICKEN STORED BELOW COOKED
FOODS IN CABINET SECTION OF IN-LINE REFRIGERATOR DURING TIME OF
RE-INSPECTION.
3-302 .11-P REGULATION: Item 15 Food separated and protected -- Packaged and
Unpackaged Food-Separation, Packaging, and Segregation
Verified 05/08/23 OBSERVATION: Observed raw shrimp and raw salmon both in sliding door
refrigerator that PIC states have not been used or taken out of unit
today at 50°F and 52°F respectively. Unit observed with significant ice
accumulation on vents. PIC started voluntarily discarding foods that have
remained in refrigerator for more than 4 hours. Discussed with PIC to
completely defrost refrigerator and cease storing any TCS foods in unit
until it is capable of maintaining food temperatures at 41°F or below.
**RE-INSPECTION** OBSERVED COOKED VEGETABLES, CHUTNEY, AND GULAB JAMUN
ALL IN SLIDING DOOR REFRIGERATOR AT 41OF AT TIME OF RE-INSPECTION.

Verified 05/08/23 OBSERVATION: No food thermometer available at time of inspection. PIC to
ensure a food thermometer is readily available and accessible to take
accurate temperatures of hot and cold foods.

RE-INSPECTION.

Verified 05/08/23 OBSERVATION: Observed presence of what appear to be phorid flies
specifically in walk-in refrigerator re-purposed as a dry storage area
off of kitchen and in dry storage area in upstairs section of
establishment. No pest control reports available on site at time of
inspection. Discussed with PIC to contact pest control company regarding
having service for flies. PIC to have pest control report at time of
re-inspection date.
**RE-INSPECTION** NO PRESENCE OF PHORID FLIES/PESTS OBSERVED IN
ESTABLISHMENT AT TIME OF RE-INSPECTION.

Verified 05/08/23 OBSERVATION: Observed what appear to be droppings consistent with mice
under receipt printer on elevated shelf above prep table across from
tandoori oven and fryers. PIC to clean and sanitize area. PIC to ensure
pest control company is treating establishment for mice and have reports
available on site for review at time of inspections.

Verified 05/08/23 OBSERVATION: Observed fly strip hanging from ceiling above warewashing
machine in kitchen area. Discussed with PIC that insect control devices
such as a fly strip shall not be installed above clean equipment.
**RE-INSPECTION** OBSERVED FLY STRIP HANGING FROM CEILING NEAR
WAREWASHING MACHINE UPON ARRIVAL TO RE-INSPECTION. UPON DISCUSSION OF
INSECT CONTROL DEVICES AND LOCATIONS WHERE THEY CAN NOT BE INSTALLED, PIC
REMOVED FLY STRIP DURING TIME OF RE-INSPECTION.

Insect Control Devices, Design and Installation
OBSERVATION: Observed utensils being stored in container of soiled, room
temperature water near stove. Discussed with PIC that if utensils are
going to be stored in water in between uses, water must be maintained at
a minimum temperature of 135°F and remain clean.
**RE-INSPECTION** OBSERVED UTENSILS BEING STORED IN ROOM TEMPERATURE
WATER AT TIME OF RE-INSPECTION. REVIEWED REQUIREMENTS FOR IN-USE UTENSIL
STORAGE WITH PIC AND EMPLOYEES.

Verified 05/08/23 OBSERVATION: Observed interior of microwave near in-line refrigerator and
underside of prep table closest to stove with food spills and dust
accumulation respectively. PIC to ensure both surfaces are cleaned at a
frequency necessary to decrease such accumulations.
**RE-INSPECTION** OBSERVED BOTH LOCATIONS MENTIONED ABOVE TO BE CLEAN AT
TIME OF RE-INSPECTION.

Verified 05/08/23 OBSERVATION: Observed pipe under three bay sink to be leaking water into
pan kept below it at time of inspection. PIC to ensure pipe is repaired
using approved methods and materials.
**RE-INSPECTION** OBSERVED PIPE UNDER THREE BAY SINK TO BE IN GOOD REPAIR
AT TIME OF RE-INSPECTION.

view. Observed cooked potatoes in top section of in-line refrigerator at
41°F. Observed cooked chicken in cabinet section of unit at 41°F.
Observed chickpeas, cooked chicken, and cooked lamb all in top section of
additional in-line refrigerator at 38°F. Observed spinach in cabinet
section of unit at 37°F. Observed samosas in walk-in refrigerator at
41°F. Observed rice in thermal rice pot at 141°F. Observed
low-temperature warewashing machine operating in compliance. Testing
strips available. Observed clean knives in knife holder. Observed
handwashing sinks in compliance.
Discussion Observed hood sticker in compliance, next service due in June
2023.
