OBSERVATION: No paper towels available at handwashing sink next to three
bay sink in back room at time of inspection. PIC to ensure handwashing
sinks are supplied with paper towels at all times.

OBSERVATION: Observed ice machine with accumulation of what appears to be
mold around chute area. PIC to ensure ice machine is emptied, cleaned,
and sanitized.
4-602 .11 REGULATION: Item 16 Food contact surface; cleaned and sanitized --

OBSERVATION: Observed table-top hot holding unit on front counter that
does not have a barrier/food guard in place to protect un-packaged
empanadas that are intended for customer self-service. PIC to ensure
un-packaged foods intended for customer self-service are protected from
potential contamination and tampering.

OBSERVATION: No food thermometer available at time of inspection. PIC to
obtain food thermometer to take accurate temperatures of hot and cold
food items.

OBSERVATION: Observed packaged sandwiches and fruit cups missing labeling
components. Discussed labeling requirements with PIC.
Label information shall include: (1) The common name of the FOOD, or
absent a common name, an adequately descriptive identity statement; (2)
If made from two or more ingredients, a list of ingredients in descending
order of predominance by weight, including a declaration of artificial
color or flavor and chemical preservatives, if contained in the FOOD; (3)
An accurate declaration of the quantity of contents; (4) The name and
place of business of the manufacturer, packer, or distributor.

OBSERVATION: Observed quaternary sanitizer testing strips with an
expiration date of May 2020. PIC to obtain new testing strips to
accurately test concentration of sanitizer.

DESCRIPTION OF VIOLATION
Fail Code
Discussion Establishment currently holds a "limited food service" food
permit but reheats frozen empanadas, hotdogs, and taquitos on site.
Manager states reheating of foods was started in establishment a few
months ago. Discussed with PIC to contact health office regarding food
permit requirements given current permit type.
Discussion Discussed with PIC to post CFPM certificates in public view.
In Compliance Observed valid CFPM certificates. Observed circular open-face
refrigerator containing commercially pre-packaged sandwiches, salads,
hard-boiled eggs, and cups of yogurt operating at 41°F. Observed
multi-glass door refrigerator containing gallons of milk operating at
41°F. Observed empanadas in table-top hot holding unit at 145°F. Observed
hotdogs and taquitos both in rolling warmer at 139°F and 141°F
respectively. Observed quaternary sanitizer available. Observed foods in
freezer units to be frozen solid. Observed dry storage in compliance.
Observed toilet rooms in compliance. No pest activity observed.
Closing Correct Priority Item and Priority Foundation Item violations immediately;
Core Item violations within 10 days. Correct all violations in entirety
and maintain. Train and supervise staff. Failure to correct all
violations and maintain corrections may result in administrative action
