vomiting and diarrheal events. Provided PIC with written procedure for
how to respond to bodily spills.

Verified 05/03/23 OBSERVATION: Observed employee remove waffle from iron with bare hands.
Discussed with PIC that food employees may not contact RTE (ready-to-eat)
foods without using a barrier such as single-use gloves, deli tissue,
tongs, spatula, etc.
**RE-INSPECTION** NO BARE HAND CONTACT WITH RTE FOODS OBSERVED AT TIME OF
RE-INSPECTION.

Verified 05/03/23 OBSERVATION: Observed bucket being stored directly inside of handwashing
sink behind front counter upon arrival to inspection. PIC to ensure
handwashing sinks are used for handwashing only and remain empty and
accessible at all times.

RE-INSPECTION.

Verified 05/03/23 OBSERVATION: No paper towels available at handwashing sink behind front
counter upon arrival to inspection. PIC supplied paper towels at sink
during time of inspection. PIC to ensure handwashing sinks are supplied
with paper towels at all times.
**RE-INSPECTION** PAPER TOWELS AVAILABLE AT HANDWASHING SINK MENTIONED
ABOVE AT TIME OF RE-INSPECTION.
6-301 .12-Pf REGULATION: Item 10 Adequate handwashing sinks properly supplied and
accessible -- Handwashing Drying Provision
OBSERVATION: Observed high-temperature warewashing machine operating at
86°F after being run for multiple cycles. PIC states service has been
called for repairing machine but unsure of date when repair will take
place. PIC voluntarily stated they will discontinue using machine and use
three bay sink to wash, rinse, and sanitize dishes and wares until
machine is repaired. PIC to send status update or repair slip to health
office within 24 hours.
**RE-INSPECTION** PER PIC, HIGH-TEMPERATURE WAREWASHING MACHINE IS NOT IN
OPERATING ORDER. PIC STATES REPAIR IS SCHEDULED FOR 5/8/2023. PIC TO SEND
REPAIR SLIP TO HEALTH OFFICE ONCE RECEIVED. THREE BAY SINK IS BEING USED
IN COMPLIANCE FOR WASHING, RINSING, AND SANITIZING UTENSILS AND WARES.
REVIEWED WITH PIC NOT TO USE WAREWASHING MACHINE UNTIL IT IS OPERATING IN
COMPLIANCE.

OBSERVATION: No irreversible registering temperature indicator available
to test high-temperature warewashing machine at time of inspection. PIC
to obtain temperature indicator to accurately test surface temperature of
warewashing machine. Discussed options of irreversible registering
temperature indicators including 160°F temperature testing strips,
stickers, dish plate thermometer, etc.
**RE-INSPECTION** NO IRREVERSIBLE REGISTERING TEMPERATURE INDICATOR
AVAILABLE TO TEST HIGH-TEMPERATURE WAREWASHING MACHINE AT TIME OF
RE-INSPECTION. PIC STATES ONE HAS BEEN ORDERED BUT UNABLE TO VERIFY. PIC
TO ENSURE INDICATOR IS OBTAINED WHEN WAREWASHING MACHINE IS OPERATING IN
COMPLIANCE ONCE REPAIRS HAVE BEEN COMPLETED.

Verified 05/03/23 OBSERVATION: Observed poached eggs on menu missing asterisks and consumer
advisory footnote. PIC to ensure foods that may be served or ordered
raw/undercooked or contain raw or undercooked ingredients (such as
hamburger, steak tips, undercooked eggs, etc. ) are identified on menu by
asterisking them to a footnote that states, "these items are served raw
or undercooked, or contain (or may contain) raw or undercooked
ingredients. "

RE-INSPECTION.

Verified 05/03/23 OBSERVATION: Observed presence of what appear to be phorid flies
specifically near warewashing machine in kitchen area. Observed latest
pest control report from Frank's Defense Pest Control dated 3/15/2023 not
noting that service for flies is currently being provided in
establishment. Discussed with PIC to contact pest control company
regarding having service for flies. PIC to have pest control report at
time of re-inspection date.
**RE-INSPECTION** NO PRESENCE OF FLIES OBSERVED AT TIME OF RE-INSPECTION.
OBSERVED LATEST PEST CONTROL REPORT FROM FRANK'S DEFENSE PEST CONTROL

OBSERVATION: Observed interior walls of high-temperature warewashing
machine in kitchen as well as dish trays used in machine with significant
accumulation of lime and what appears to be mold. PIC to de-lime and deep
clean interior of machine and replace dish trays that are in poor
condition.
**RE-INSPECTION** OBSERVED INTERIOR WALLS OF WAREWASHING MACHINE IN SAME
CONDITION AS MENTIONED ABOVE. OBSERVED DISH TRAYS SPECIFICALLY TO LEFT OF
MACHINE ON SHELF WITH ACCUMULATION OF SOIL RESIDUES. REVIEWED WITH PIC TO
DE-LINE AND DEEP CLEAN INTERIOR OF MACHINE AND REPLACE DISH TRAYS. WILL
VERIFY COMPLIANCE AT NEXT ROUTINE INSPECTION.

Verified 05/03/23 OBSERVATION: Observed interiors and exteriors of non-food contact surfaces
with accumulation. Clean surfaces of all non-food contact surfaces
including but not limited to shelves in walk-in refrigerator, interior
surfaces of two door low-boy freezer behind front counter, and cabinet of
in-line refrigerator.
**RE-INSPECTION** OBSERVED IMPROVED CLEANLINESS OF SURFACES AT TIME OF
RE-INSPECTION.

Verified 05/03/23 OBSERVATION: Due to number and nature of violations cited above and
evidence of overall poor cleaning practices suggestive of poor managerial
control, it appears PIC is not performing their duties.

Discussion Discussed with PIC to post CFPM and allergen certificates in
public view.
**RE-INSPECTION** CFPM AND ALLERGEN CERTIFICATES POSTED IN PUBLIC VIEW AT
TIME OF RE-INSPECTION.
In Compliance Observed valid copies of both CFPM and allergen certificates.
Observed diced tomatoes and ham both in top section of in-line
refrigerator at 41°F. Observed breakfast potatoes in cabinet section of
unit at 38°F. Observed container of sausage links kept on ice near grill
at 41°F. Observed breakfast potatoes, bacon, and chicken stock all in
walk-in refrigerator at 35'F, 41°F, and 41°F respectively. Observed
quaternary sanitizer testing strips available. Observed food thermometer
available. Observed clean ice machine. Observed toilet rooms in
compliance.
Discussion Observed hood sticker in compliance, next service due in
September 2023.
Closing Violations marked "Verified" have been corrected. Violations not marked
"Verified" remain uncorrected. Uncorrected violations are to be corrected
immediately. Uncorrected violations may result in additional
Re-inspections and fees, fines and or administrative action including
